Akut Koroner Sendromu Bulunan Obez Türk Hastalarda Leptin, Rezistin ve PPAR-Gama Gen Varyantlarının Etkileri

Amaç: Obezite ve akut koroner sendrom (AKS) son yılların yaygın sağlık problemlerindendir. AKS ve obezitenin genetik altyapısı ile ilgili bir çok aday gen bildirilmektedir. Bu çalışmanın amacı, Türk popülasyonundaki AKS’li obez hastalarda Leptin glutamin-arginin substitution (Gln223Arg), Rezistin -420 Sitozin/Guanin (C/G) ve proliferatör ile aktive edilen reseptör gama (PPAR-γ) prolin-alanin substitution (Pro12Ala) polimorfizmlerinin etkilerini incelemektir. Yöntemler: Çalışmaya AKS tanısı konmuş 50 obez hasta ve 42 sağlıklı kontrol dahil edilmiştir. Polimorfizmler polimeraz zincir reaksiyonu-restriksiyon parça uzunluk polimorfizmi (PZR-RFLP) ve agaroz jel elektroforezi metotları kullanılarak analiz edildi. Bulgular: PPAR-γ Pro12Ala polimorfizmi için Pro/Ala (p=0,001) genotipi taşıma oranı hasta grubunda kontrol grubuna göre, Proline/Proline (Pro/Pro) genotipi (p=0,001) taşıma oranı ise hasta grubuna oranla kontrol grubunda anlamlı düzeyde daha yüksektir. Rezistin -420 C/G polimorfizminin GC genotip (p=0,045) dağılımı ise hasta grubu ile kıyaslandığında kontrol grubunda anlamlı olarak yüksek bulundu. Sonuç: Çalışmamız, obez bireylerde AKS için PPAR-γ Pro12Ala polimorfizminin Pro/Ala genotipinin bir risk faktörü, Rezistin -420 C/G polimorfizminin GC genotipinin ve PPAR-γ Pro12Ala polimorfizminin Pro/Pro genotipinin ise koruyucu bir faktör olabileceğine dair yeni bulgular sağlamaktadır.

Obesity and acute coronary syndrome (ACS) are among the common health problems of recent years. Many candidate genes relating to the genetic infrastructure of ACS and obesity are. The aim of this study is to study the effects of Leptin glutamin-argin substitution (Gln223Arg), Resistin -420 Sitozin/Guanin (C/G) and proliferator-activated receptor gamma (PPAR-γ) prolin-alanin substitution (Pro12Ala) polymorphisms in the Turkish population. Methods: The study included 50 obese patients diagnosed with ACS and 42 healthy controls. Polymorphisms were analyzed using polymorphism length-restriction particle reaction-reaction chain (PZR-RFLP) and agarose gel electrophoresis methods. For PPAR-γ Pro12Ala polymorphism, the Pro/Ala (p=0,001) genotype transfer rate is significantly higher in the control group, while the Proline/Proline (pro/Pro) genotype transfer rate (p=0,001) is significantly higher in the control group than the patient group. The GC genotype (p=0,045) distribution of resistant -420 C/G polymorphism was significantly high in the control group compared to the patient group. Result: Our study provides new findings that PPAR-γ Pro12Ala polymorphism is a risk factor of the Pro/Ala genotype for ACS in obese individuals, Resistant -420 C/G polymorphism can be a protective factor of the GC genotype and PPAR-γ Pro12Ala polymorphism can be a pro/Pro genotype.
