Küreselleşme ve küreselleşmenin getirdiği özerkleşme ve piyasa güdümlü hizmet anlayışı örgütsel yapı ve hizmet sunumu üzerinde etkili olmuştur. Özellikle 1980’li yıllarından sonra Türkiye’de Sağlık Bakanlığı merkezi ve taşra örgüt yapısı, hizmet sunumu ve istihdam modelleri gibi uygulamalarda bir arayış içinde olmuştur. Bu arayış çabaları genellikle Kanun Hükmünde Kararname ile uygulama alanı bulmuştur. Ancak KHK ile yapılan düzenlemeler normal kanunlar gibi bir süreç izlememekte ve Bakanların alacağı kararla bir anlık ihtiyaçtan dolayı uygulamaya konmaktadır. Bu bakımdan yapılan uygulamalar derinlemesine düşünülmeden hayata geçirilerek birtakım olumsuzluklara neden olabilmektedir. Buradan hareketle çalışmada, Sağlık Bakanlığı’nın görev ve örgütsel yapılanmasının merkezi ve yerel düzeyde 181, 663 ve 694 sayılı kararnameler ışığında yaşanan değişim ve dönüşümler, önemli görülen bazı maddeler ışığında irdelenmesi amaçlanmıştır. Çalışmada kararnamelerin uygulanmasıyla meydana gelen sorun ve kazanımlar, olası eleştiriler ve süreçte yaşanan değişimler ile gelinen son noktaya ışık tutularak otoritelere öneriler geliştirilmiştir.
The concept of autonomy and market-driven service brought by globalization and globalization has influenced the organizational structure and service provision. Especially after the 1980s, the Ministry of Health in Turkey has been in a search for applications such as the center and organizational structure, service presentation and employment models. These search efforts have often found the scope of application with the Charter in the Law. However, the regulations made with the KHK do not follow a process like normal laws and the ministers will make a decision to apply due to an immediate need. In this regard, the practices made without deep consideration can lead to a number of negative effects. From here in work with the move, the changes and transformations that occur in the light of the decrees 181, 663 and 694 at the central and local level of the Ministry of Health’s mission and organizational structure are intended to be reflected in the light of some important elements. In the study, the problems and achievements arising from the implementation of the decrees, the possible criticisms and the changes in the process, the final point of lighting, the authorities have developed recommendations.
