Covid-19 salgın süreciyle birlikte toplumun özellikli gruplarından olan kanser hastaları sağlık hizmetlerine erişimle ilgili ciddi kaygılar yaşamış ve genellikle ileri yaş grubunda olan, kronik hastalığı bulunan ve immünosüpresyon tedavi alan bu hastalar, Covid-19 salgınına karşı daha savunmasız ve riskli duruma gelmiştir. Bu bağlamda, kanser hastalarının mevcut tedavilerinin aksatılmadan sürdürülmesi ve beraberinde Covid-19 salgınının olumsuz etkilerinden korunması ayrı bir organizasyon gerektirmiştir. Bu çalışmanın amacı, Covid-19 salgın sürecinde kanser hastalarının sağlık hizmetine erişim ve kullanım durumlarına yönelik geçerli ve güvenilir bir ölçek geliştirmektir. Çalışma, bir üniversite hastanesinin ayaktan kemoterapi ünitesinde Nisan 2021-Nisan 2022 tarihleri arasında toplam 769 kanser hastası üzerinde gerçekleştirilmiştir. Araştırmacı tarafından 45 maddelik soru havuzu oluşturulmuş ve uzman görüşü sonrası 9 madde kapsam dışı bırakılarak 36 maddelik taslak ölçek elde edilmiştir. Veri analizleri SPSS 23 ve SPSS AMOS programı kullanılarak gerçekleştirilmiştir. İstatistiksel yöntem olarak açımlayıcı ve doğrulayıcı faktör analizleri, test-tekrar test yöntemi ve iç tutarlılık analizi kullanılmıştır. Taslak ölçeğin Kaiser-Meyer-Olkin (KMO) değeri 0,870 Barlett-testi sonucu 5513,998 (p<0,000) bulunmuş ve verilerin açımlayıcı faktör analizine (AFA) uygun olduğu sonucuna ulaşılmıştır. Açımlayıcı faktör analizi sonucunda 4 faktörlü ve 19 maddelik ölçek elde edilmiştir. Açıklanan toplam varyans 59,567 ve maddelerin faktör yük değerleri 0,640 ile 0,895 arasında hesaplanmıştır. Doğrulayıcı faktör analiziyle (DFA) faktör yapısının uyumluluğu teyit edilmiş ve modelin oluşturulan faktörlerle uyumlu olduğu görülmüştür. Alt-üst gruplara dayalı madde analizi sonucunda maddelere ait t-testi sonucu anlamlı bulunmuştur (p<0,001). Ölçeğin Cronbach Alpha katsayısı 0,811 hesaplanmış ve test-tekrar test sonucunda (n=58) iki test arasındaki tutarlılığın yüksek düzeyde olduğu görülmüştür (r=0,995, p<0,05). Sonuç olarak geliştirilen ölçeğin, Covid-19 salgın sürecinde kanser hastalarının sağlık hizmetine erişim ve kullanım durumlarını belirlemede geçerli ve güvenilir olduğu sonucuna varılmıştır.
With the Covid-19 epidemic process, cancer patients from specific groups of the community experienced serious concerns about access to health care and these patients, often in the older age group, with chronic illness and receiving immunosuppression treatment, became more unprotected and risky against the Covid-19 epidemic. In this context, a separate organization was required to continue the existing treatments of cancer patients without interruption and to protect them from the adverse effects of the Covid-19 epidemic. The aim of this study is to develop a valid and reliable scale for the conditions of access and use of health care for cancer patients during the Covid-19 epidemic process. The study was conducted on a total of 769 cancer patients in a university hospital's foot-to-foot chemotherapy unit between April 2021 and April 2022. The researcher created a 45-matter question pool and after expert opinion, 9 matters were excluded and 36-matter draft scale was obtained. Data analysis was carried out using SPSS 23 and SPSS AMOS programs. As a statistical method, detection and verifying factor analysis, test-up test method and internal consistency analysis have been used. The draft scale value of Kaiser-Meyer-Olkin (KMO) was 0.870 Barlett-test resulted in 5513,998 (p<0,000) and the result was that the data were in accordance with the AFA analysis. The opening-factor analysis resulted in 4 factor and 19 material scales. The total variance described is 59.567 and the factor load values of the materials are calculated between 0.640 and 0.895 . The verifying factor analysis (DFA) has confirmed the conformity of the factor structure and the model has been found to be compatible with the factors created. The substance analysis based on the sub-top groups found the substance t-test result meaningful (p<0,001). The Cronbach Alpha ratio of the measurement was calculated at 0.811 and the test-up test resulted in a high level of consistency between the two tests (n=58) (r=0.995, p<0.05). As a result, the scale developed was found to be valid and reliable in determining the conditions of access and use of health care for cancer patients during the Covid-19 epidemic process.

Data analyzes were performed using SPSS 23 and SPSS AMOS program. Exploratory and confirmatory factor analyses, test-retest method and internal consistency analysis were used as statistical methods. The Kaiser-Meyer-Olkin (KMO) value of the draft scale was found to be 0.870 Barlett-test result of 5513,998 (p<0.000) and it was concluded that the data were suitable for exploratory factor analysis (EFA). As a result of exploratory factor analysis, a 4-factor and 19-item scale was obtained. The total variance explained was 59.567 and the factor loadings of the items were calculated between 0.640 and 0.895. The compatibility of the factor structure was confirmed by confirmatory factor analysis (CFA) and it was seen that the model was compatible with the factors created. As a result of the item analysis based on the lower and upper groups, the t-test result of the items was found to be significant (p<0.001). The Cronbach Alpha coefficient of the scale was calculated as 0.811 and as a result of the test-retest (n=58), it was seen that the consistency between the two tests was high (r=0.995, p<0.05). As a result, it has been concluded that the developed scale is valid and reliable in determining the access and use of health care services of cancer patients during the Covid-19 pandemic process.
