Amaç: Bu çalışmanın amacı 2014-2018 yılları arasında alt ekstremitede doku defekti nedeni ile flep rekonstrüksiyonu uygulanan ve post-operatif yoğun bakım ünitesinde (YBÜ) takip edilen hastalarda post-operatif flep komplikasyonu ile ilişkili faktörlerin değerlendirilmesidir. Gereç ve Yöntem: Hastaların demografik verileri, doku defektinin boyutu, hemoglobin düzeyi, anestezi ve cerrahi süreleri, kan transfüzyon miktarları, post-operatif komplikasyon varlığı, hastanede yatış süreleri, doku kültür sonuçları ve YBÜ’de yatış süreleri retrospektif olarak kayıt altına alındı. İstatistiksel analiz için SPSS programı kullanıldı. Bulgular: Doku defektinin en sık nedeni trafik kazasıydı ve defekt boyutu 118,35±74,74 cm2 idi. Post-operatif komplikasyonun 12 (%42,9) hastada geliştiği gözlemlendi. En sık gelişen komplikasyonlar; hematom ve parsiyel flep nekrozuydu. On hastanın (%35,7) kültür sonuçlarında üreme olduğu saptandı. Flep komplikasyonu gelişen hastalarda, gelişmeyenlerle karşılaştırıldığında, sigara kullanımı, Amerikan Anestezi Derneği (ASA) skoru ve yaş anlamlı olarak daha yüksekti (p<0,05). YBÜ’de yatış süreleri benzerdi. Post-operatif hastanede yatış süreleri bu hastalarda daha uzundu (p<0,05). Sonuç: Çalışmamızda, post-operatif flep komplikasyon gelişimi açısından sigara kullanımı, yaş ve ASA skorunun önemli birer risk faktörü olduğu görüldü. Bu hastalarda post-operatif hastanede yatış süresinin daha uzun olduğu belirlendi.
Purpose: The aim of this study is to evaluate the factors associated with post-operative flep complication in patients who are undergoing flep reconstruction and followed in post-operative intensive care unit (PCI) between 2014-2018 years due to tissue defects in the lower limbs. Instruments and Methods: The demographic data of patients, the size of tissue defect, the level of hemoglobin, anesthesia and surgical periods, blood transfusion amounts, the presence of postoperative complications, hospitalization periods, tissue culture results and hospitalization periods in the UBS were recorded retrospectively. The SPSS program was used for statistical analysis. Results: The most common cause of tissue defect was a traffic accident and the defect size was 118.35±74.74 cm2. Postoperative complications were observed in 12 (42.9%) patients. The most common complications were hematom and parcial flep necrosis. Ten patients (35.7%) were found to be reproductive in cultural results. In developing patients with Flep complication, compared to those undeveloped, smoking was significantly higher than the American Anesthesia Association (ASA) score and age (p<0,05). It was similar to the time in YBU. The postoperative hospitalization periods were longer in these patients (p<0,05). The result: In our study, it was found that smoking, age and ASA score were an important risk factor in terms of post-operative flep complications. These patients have been diagnosed with a longer postoperative hospital stay.
