Abstract:

Savunmacı tıp, sorumluluk endişesi nedeniyle hekimlerin standart tıbbi uygulamadan sapması olarak tanımlanmaktadır. Hekimler iki farklı şekilde savunmacı bir yaklaşım ortaya koyabilirler. Bunlar; pozitif savunmacı tıp yaklaşımı, negatif savunmacı tıp yaklaşımıdır. Hastaya ya da hastalığa pozitif savunmacı tıp bakış açısıyla yaklaşan hekimler kendilerini güvenceye almak için hastaların ihtiyacı olmayan sağlık hizmetlerini sunabilmektedir. Negatif savunmacı tıp yaklaşımında ise hekimler hastayı tedavi etmeme ya da tedaviyi geciktirme eğilimindedir. Hekimleri bu türden davranışlara yönelten başlıca nedenler ise tıbbi davalara konu olma, tazminat ödeme ya da itibar kaybı gibi endişelerdir. Hekimlere yönelik malpraktis suçlamalarına karşı olarak organizasyon düzeyinde klinik uygulama rehberlerinin kullanılması, kanıta dayalı tıp uygulamalarının organizasyon düzeyde yaygınlaştırılması, malpraktis sigortasının teşviki, çağdaş hasta-hekim işbirliği ve tıbbi kayıtların düzenli tutulması konularında kültürel bir dönüşüm sağlanabilir. Bu çalışmanın amacı sağlık sektöründe hekimler arasında giderek artan bir endişe kaynağı olan savunmacı tıp konusunu nedenleri ile birlikte incelemek ve buna dayalı olarak birtakım önerilerde bulunmaktır.

Abstract:

Defence medicine is defined as a deviation of doctors from standard medical practice due to responsibility concerns. Doctors can present a defensive approach in two different ways. These are the positive defence approach, the negative defence approach. Doctors who approach the patient or the disease from a positive defensive medical point of view can provide patients with unnecessary health care to secure themselves. In the negative defensive approach to medicine, doctors tend to not treat the patient or delay the treatment. The main reasons that direct the doctors to such behaviors are concerns such as being subject to medical cases, paying compensation or losing reputation. A cultural transformation can be achieved in the fields of the use of clinical practice guidelines at the organizational level against the accusations of malpracticism against doctors, the spread of evidence-based medical practices at the organizational level, the promotion of malpractic insurance, contemporary patient-doctoral cooperation and the regular maintenance of medical records. The aim of this study is to study the issue of defensive medicine, which is a growing source of concern among doctors in the health sector, along with the reasons and on the basis of it to make a number of recommendations.
