Testicular sarcomas are rare tumors that may be confused with other testicular malignancies. Liposarcoma is the most common type of primary paratesticular sarcomas. An 83-year-old male presenting with painless left testicular swelling underwent an orchiectomy upon a provisional diagnosis of testicular tumor. The microscopic examination revealed a greyish-white tumoral structure of medium hardness, measuring 7.4 cm in diameter and covering the entire left testicle to eliminate the normal testicular tissue. Further microscopy showed enlarged pleomorphic cells that are round, oval, or spindle-shaped with hyperchromatic nuclei, prominent nucleoli in a fibromyxoid stroma, as well as binuclear and multinuclear giant cells and an infiltration consisting of lipoblasts within adipose tissue. Based on these findings, the patient was diagnosed with primary paratesticular pleomorphic liposarcoma. The case has been presented for discussion, as it is a rare disease with difficulties in differential diagnosis.
