Kolorektal kanser (KRK) dünya genelinde yıllık 1 milyonun üzerinde yeni vaka ile en yaygın olarak görülen üçüncü kanser türüdür. Türkiye’de 2010 yılı verileri itibariyle KRK insidansı erkeklerde 20,7/100.000, kadınlarda 13,1/100.000’dir. Cinsiyete göre sahip olunan bu oranlar ile KRK erkeklerde dördüncü sırada, kadınlarda ise üçüncü sırada en yaygın görülen kanser türleri olarak belirlenmiştir. Son yıllarda dünya genelinde ve Türkiye’de sağlık harcamalarında ciddi artışlar meydana gelmiştir. Sağlık harcamalarında meydana gelen bu artış tıbbi teşhis ve tedavi maliyetinin yanı sıra hastalıklar nedeniyle oluşan iş gücü kaybı ve yaşam kalitesinin düşmesi gibi birçok faktörden kaynaklanmaktadır. Sağlığa ayrılan kaynakların sınırlı olması nedeniyle ülkeler için yüksek düzeyde hastalık yüküne neden olan özellikle KRK gibi hastalıkların maliyetleri konusunda çalışmalar yapılması birçok ülke için oldukça önemlidir. Çalışmanın amacı kolon ve rektum kanseri tedavi maliyetinin yukarıdan aşağı maliyet yaklaşımı ile SGK perspektifinden hesaplanmasıdır. Yukarıdan aşağı maliyet yaklaşımı ile kolon ve rektum kanseri hastalarının SGK perspektifinden tedavi maliyetini belirlemeye yönelik olarak T.C. Sosyal Güvenlik Kurumu (SGK) MEDULA Sistemi’nden Türkiye genelinde faaliyet gösteren SGK ile anlaşması olan tüm hastanelerde Ocak – Aralık 2014 dönemi arasında kolon ve/veya rektum kanseri tanısı ile tedavi hizmetlerinden yararlanan hastaların verileri elde edilmiştir ve toplam 62.970 hasta verisi üzerinden gerekli hesaplamalar yapılmıştır. Yukarıdan aşağı maliyet yaklaşımı ile kolon kanseri hastalarının ortalama hasta başı maliyeti 3.055,5TL, rektum kanseri hastalarının 4.146,2TL olarak hesaplanmıştır. Metastatik kolon ve rektum kanseri hastalarının ortalama hasta başı maliyetinin metastatik olmayan hastalara kıyasla oldukça yüksek olduğu tespit edilmiştir. Ortalama hasta başı maliyet metastatik kolon kanseri hastaları için 5.153,8TL, metastatik olmayanlar için 2.629,4TL, metastatik rektum kanseri hastaları için 6.618,2TL, metastatik olmayan hastalar için 3.802,1TL olarak hesaplanmıştır.
Colorectal cancer is the third most common type of cancer worldwide with over 1 million new cases annually. In Turkey, according to 2010 data, the KRK incidence is 20.7/100.000 in men and 13.1/100.000 in women. This is the fourth most common type of cancer in men and the third most common type of cancer in women. In recent years, there has been a significant increase in health expenditure throughout the world and in Turkey. This increase in health expenditure is due to many factors such as medical diagnosis and treatment costs as well as the loss of labor and the decrease in the quality of life caused by diseases. Due to the limited resources dedicated to health, research on the costs of diseases, especially CCC, which cause a high level of disease burden for countries, is very important for many countries. The purpose of the study is to calculate the cost of the treatment of colon and rectum cancer with the cost approach from above to below from the SGK perspective. T.C. with a cost approach from above to below to determine the treatment costs of colon and rectum cancer patients from the SGK perspective. In all hospitals that have agreed with the SGK operating throughout Turkey from the Social Security Agency (SGK) MEDULA System from January to December 2014, the data of patients who have been diagnosed with colon and/or rectum cancer and received treatment services have been obtained and a total of 62. There are 970 patient data. With the cost approach from above to below, the average cost per patient for colon cancer patients is estimated at 3.055,5TL, and the rectum cancer patients are estimated at 4.146,2TL. The average cost per patient for metastatic colon and rectum cancer patients has been found to be quite high compared to non-metastatic patients. The average cost per patient is estimated at 5.153,8TL for metastatic colon cancer patients, 2.629,4TL for non-metastatic patients, 6.618,2TL for metastatic rectum cancer patients, 3.802,1TL for non-metastatic patients.
