Abstract:

Sağlık sisteminin pek çok çıktısı hekim-hasta iletişiminin niteliğinden yakından etkilenmektedir. Hekim tatmini, hasta memnuniyeti, hemşire ve diğer sağlık personelinin memnuniyeti hekim-hasta iletişiminden etkilenen unsurlardan bazılarıdır. Bu kadar çok çıktıyı etkileyen bir konunun daha derinlemesine incelenmesi gerekildiği düşünülerek bu araştırma kapsamında hekim ve hasta iletişimini inceleyen bir ön çalışma yapılmıştır. Yazın incelendiğinde hekimlerin tıbbi terminolojiyi kullanmasının hekim-hasta iletişimini olumsuz yönde etkilediğini ortaya koyan pek çok çalışma göze çarpmaktadır. Ancak sorunun daha açık bir biçimde ortaya konması için öncelikle soruna her iki taraf açısından bakmak daha yararlı olacaktır. Bu doğrultuda söz konusu araştırmada öncelikle hekimlerin ve hastaların karşılıklı olarak bir iletişim sorunu yaşayıp yaşamadıkları saptanmaya çalışılmıştır. Öte yandan, varsa eğer, böyle bir sorunun kimden kaynaklanıyor olabileceği, tarafların bu tür sorunları çözdükleri gibi sorulara da yanıt aranmıştır. Araştırmada projektif (analitik) görüşme yöntemi kullanılarak hekim-hasta iletişimi üzerinde nitel bir ön çalışma yapılmıştır. Kolayda örneklem yöntemiyle seçilen 10 hekim ve 10 hasta olmak üzere toplamda 20 katılımcı ile yüz yüze görüşülmüştür. Katılımcılara önceden hazırlanmış farklı soru gruplarının yöneltildiği görüşmeler sonucunda iletişim sorununun kaynağı, nedenleri ve tarafların bu sorunla baş etme yolları hakkında bilgi toplanmıştır. Ayrıca yapılan görüşmeler sonucunda ataerkil hekim-hasta ilişkisinden daha demokratik bir ilişkiye doğru gidildiği anlaşılmıştır. A QUALITATIVE PRE-STUDY ON THE PHYSICIAN-PATIENT COMMUNICATION ABSTRACT Many outputs such as physician satisfaction or patient satisfaction are closely affected by the quality and qualification of doctor-patient communication. Such an important issue that affects so many outputs should be examined in more depth in order to figure out the problems of doctor-patient satisfaction which is why this pre-study was applied on doctor-patient communication. Many studies in the literature show that the main effect on doctor-patient communication is the usage of medical terminology. But in order to understand the problem much clearly, it will be better to analyze it from both sides' view. Thus one of the main aims of this pre-study was to find out if there is really a communication problem as well as trying to find out the answers to the questions like the sources of this problem and how both sides try to deal with it. This qualitative pre-study was done using projective (analytical) method by asking several questions prepared earlier to the 20 (10 physicians and 10 patients) participants which were chosen by convenience sampling method. According to the results one can say that there's a transition from paternalistic doctor-patient relation to a more democratic one. Abstract:

Many outcomes of the health system are closely affected by the nature of the doctor-patient communication. The satisfaction of the doctor, the patient satisfaction, the satisfaction of the nurse and other health staff are some of the elements affected by the doctor-patient communication. Considering that a topic that affects so many outcomes needs to be studied more in depth in this study, a preliminary study was conducted to study the communication between the doctor and the patient. Many studies show that the use of medical terminology by doctors during the summer study has a negative impact on the doctor-patient communication. But it would be more useful to look at the problem from both sides in the first place to make the problem clearer. In this regard, the study first tried to determine whether doctors and patients experienced a communication problem with each other or not. On the other hand, if there is, the question is asked for answers, as if the parties solve such problems, which could be the source of such a problem. In the study, a qualified preliminary study on the doctor-patient communication was conducted using the proactive (analytic) conversation method. In total, 20 participants, including 10 doctors and 10 patients, were interviewed face-to-face by easy sampling method. As a result of the discussions, the participants were addressed to different group of questions prepared in advance, information was collected about the source of the communication problem, the causes and the ways the parties to deal with this problem. It is also clear that the talks were conducted towards a more democratic relationship than the patriotic doctor-patient relationship.
