Acil Serviste Uygulanan Tüp Torakostomi Girişimlerinin Değerlendirilmesi

Abstract:

Amaç: Bu çalışmada, acil tıp kliniğinde tüp torakostomi uygulanan hastalarda demografik ve klinik özelikler ile komplikasyonların incelenmesi amaçlandı. Yöntem: Retrospektif olarak yapılan bu çalışmaya Sağlık Bilimleri Üniversitesi Şişli Hamidiye Etfal Eğitim ve Araştırma Hastanesi Acil Tıp Kliniği’nde 2 yıllık süreçte acil tıp hekimleri tarafından tüp torakostomi uygulandığı tespit edilen 263 hasta dahil edildi. Tüp torakostomi uygulanmış hastaların demografik özellikleri, tüp torakostomi uygulanma tanıları, oluş mekanizmaları, morbidite ve komplikasyonlar incelendi. Bulgular: Çalışmaya dahil edilen hastaların yaş ortalaması 35,89 ± 17,47 olup hastaların 224’ü (%85,2) erkek, 39’u (%14,8) kadın idi. Acil serviste tüp torakostomi uygulama endikasyonları olarak 174 hastada pnömotoraks, 74 hastada hemopnömotoraks, 10 hastada hemotoraks ve 5 hastada ampiyem/efüzyon mevcut idi. Tüp torakostomi uygulanan hastalarda endikasyonları ortaya çıkaran etyolojik mekanizmalar incelendiğinde; 152 hastada (%58) non-travmatik, 111 hastada (%42) travmatik nedenler saptandı. Ek olarak, non-travmatik nedenler olarak sıklık sırasına göre 134 hastada spontan pnömotoraks, 12 hastada iyatrojenik ve 5 hastada komorbit ve enfeksiyonlara sekonder gelişen ampiyem/efüzyon mevcut idi. Tüp torakostomiye bağlı yalnızca 8 hastada (%3) komplikasyon geliştiği saptandı. Sonuç: Acil serviste tüp torakostomi uygulama endikasyonları en sık pnömotoraks nedeniyle olup etiyolojide non-travmatik nedenler başı çekmektedir. Ayrıca, tüp torakostomi girişimlerinin acil tıp hekimleri tarafından uygulanmasının komplikasyon arttırıcı bir faktör olmadığı ve işlemin komplikasyon oranının %3 gibi öngörülebilir düzeyde olduğu bulunmuştur.

Evaluation of the Toracostomy Pipe Initiatives Applied in Emergency Service

Purpose: This study was aimed at the study of complications with demographic and clinical characteristics in patients with pipe toracostomy in the emergency medical clinic. Method: Retrospective this study was included in 263 patients identified that pipe toracostomy was applied by emergency medical doctors in the 2 year process at the University of Health Sciences Bottle Hamidiye Etfal Education and Research Hospital. The demographic characteristics of patients applied to tube toracostomy, the diagnosis of application to tube toracostomy, the mechanisms of formation, morbidity and complications were studied. Results: The average age of the patients involved in the study was 35.89 ± 17.47 and 224 of the patients (85.2%) were male and 39 (14.8%) female. As indications for the application of tube toracostomy in emergency service, 174 patients had pnömotoraks, 74 patients had hemopnömotoraks, 10 patients had hemotoraks, and 5 patients had ampym/fusion. When the ethological mechanisms that reveal indications were studied in patients with tube toracostomy; 152 patients (58) were non-traumatic, 111 patients (42) were traumatic causes. In addition, 134 patients were spontaneous pneumotorax, 12 were iatrogenic, and 5 were comorbitic and infectious, with a secondary development of ampion/fusion. It was found that only 8 patients (3%) developed complications related to tubular toracostomy. The result: the indications for the application of tube toracostomy in emergency service are most often caused by pnömotoraks and are not traumatic causes in etiology. Furthermore, the application of tube toracostomy initiatives by emergency physicians has not been a complication factor, and the complication rate of the process is at a predictable level of 3%.
