Konvansiyonel Kron Boyu Uzatma Operasyonu Uygulanamayan Dişlere Yeni Bir YaklaşımFatih ARIKAN, Nihan KARAKURUM, Orhun BENGİSU Ege Üniversitesi, Dişhekimliği Fakültesi, Periodontoloji Ad, İzmir Üst santral dişlerindeki metal destekli porselen kuronlarının sıkça düşmesi sebebi ile kliniğimize gelen 13 yaşındaki kadın hastanın yapılan klinik ve radyografik muayenesinde bu dişlerine endodontik kanal postlarının yerleştirilmiş olduğu saptandı. Kalan sağlam diş dokularının dişetinin 3 mm kadar apikalinde olduğu görüldü ve kron boyu uzatma operasyonunun dişeti seviyesini yandaki dişlerle uyumsuz hale getirerek estetik sorun doğurabileceğine karar verildi. Bu nedenle cerrahi ekstruzyon tekniği tercih edilerek dişleri eski konumlarından daha koronerde pozisyonlandırması ve bu konumda sabitlemesini sağlamak amacıyla periodontal ligamentlerine hasar vermemeye dikkat edilerek çekilen dişler hem sağ-sol, hem de vestibül-palatinal olarak yer değiştirildi. Dişler gingival marjinin 1 mm üzerinde konumlandırılarak yeterli biyolojik aralık ve kuron için retansiyon sağlamak amaçlandı. Bu şekilde dişler sabitlenip iyileşme sonrası kompozit post-korları yapıldı, 6. ayda yeni metal destekli porselen kuronları tamamlandı. 2 sene takip sonrasında dişlerin lamina duraları radyografik olarak sağlıklı olduğu gözlendi. Konvansiyonel transplantasyon bu olgularda başarılı bir tedavi alternatifi olabilir.
A new approach to the unapplicable tooth is found by Fatih ARIKAN, Nihan KARAKURUM, Orhun BENGİSU Ege University, Faculty of Dentistry, Periodontology Ad, Izmir Upper Central Teeth with a frequent fall of metal-supported porselen curons in our clinic by a 13-year-old female patient in the clinical and radiographic examination that endodontic channels are placed on these teeth. The remaining solid tooth tissues were found to be up to 3 mm apical in the tooth, and it was decided that the chronic extension operation could cause aesthetic problem by disconnecting the tooth level with the side teeth. Therefore, the surgical extrusion technique was preferred to position the teeth in more coronaries than the old positions and to ensure that it is fixed in this position, and the periodontal ligaments were not damaged by the dragged teeth were replaced both right-left and vestibular-palatinal. The objective was to provide retention for sufficient biological range and coronation by placing the teeth above 1 mm of the gingival margin. In this way, the teeth were fixed and the composite post-cores were made after healing, in the 6th month the new metal-backed porselen curons were completed. After two years of follow-up, the lamina stations of the teeth were observed as radiographically healthy. Conventional transplantation can be a successful alternative to treatment in these cases.
