Abstract:

Purpose: The purpose of this study is to evaluate the effectiveness of the treatment of percutaneous transkate anjioplasty (PTA) applied to infrapopliteal arteries with peripheral arterial disease (PAH). Instruments and Methods: between January 2018 and January 2019, the infrapopiletal PAH cause and endovascular PTA treatment were recorded in patients and the control Doppler ultrasound (DUSG) reports were obtained. The DSA images recorded before and after treatment were examined and listed which vascular structures were ill, the number and the response to treatment. Patients with critical foot ischemic, ischemic resting pain or foot wounds, endovascular treatment were included in the study. As the criteria for technical success of endovascular therapy, the achievement of the normal clarity of the disease vascular structure in the DSA was determined as sufficient for the transition of the contrast substance to this level of distal. Control in DUSG applications was determined as a patent criterion to be the triphic sub-end arterial flow form in normal form. After the first treatment, one year of open stay without the need for other endovascular treatment was considered primary patents. In this process, if additional treatment was needed to keep the vascular open, it was regarded as a secondary patent. Results: In the total endovascular treatment, data of 19 patients with 27 infrapopliteal artery diseases were included in the study. In 15 patients (78.9 percent), nine (47.3 percent) had diabetes and seven (36.8 percent) had a history of smoking. 68.4 percent of patients experienced ischemic skin ulcers and 42 percent had rest pain. There was a pain symptom that occurred with exercise. There was no major complication that required surgery in any patient after the PTA applied. The annual primary patent ratio was 73.6 percent (14/19), and the secondary patent ratio was 84.2 percent (16/19). Intrapopliteal PAH treatment, endovascular PTA can be used safely with low complications and high patent rates.
