Ülkelerin sağlık göstergeleriyle gelişmişlik düzeyleri arasında yakın ilişki bulunması, sağlık harcamalarının gayrisafi yurt içi hasıla içindeki payının yüksek olması ve sağlık hizmetlerine olan talebin artması, kaynakların etkin kullanımıyla ilgili endişelerin ortaya çıkmasına neden olmuştur. Hükümetlerin temel hedeflerinden olan etkin çalışan bir sağlık sisteminin oluşturulabilmesi için sağlık hizmetlerinin etkinliğinin ve verimliliğinin sistematik bir şekilde değerlendirilmesi ve etkin çalışmayan alanların saptanarak, iyileştirmelerin yapılması gerekmektedir. Türkiye’deki sağlık hizmetlerinin makro düzeyde ele alındığı bu çalışmada 2016 yılı için il bazında sağlık hizmetlerinin etkinliğinin ölçülmesi ve 2012-2016 dönemi için illerin sağlık hizmetleri verimliliğindeki değişimin saptanarak, radyal ve radyal olmayan Veri Zarflama Analizi (VZA) ve Malmquist Verimlilik Endeksi (MVE) modellerinden elde edilen bulguların karşılaştırmalı olarak incelenmesi amaçlanmıştır. Türkiye’deki iller sosyo-ekonomik gelişmişlik düzeylerine göre üç gruba ayrılmış ve analizler her bir grup için ayrı ayrı gerçekleştirilmiştir. Analiz sonucunda radyal ve radyal olmayan modeller arasında yüksek ve anlamlı ilişki bulunduğu ve yöntemlerin monoton bir şekilde bağlantılı olduğu tespit edilmesine rağmen, aylak değişkenleri ihmal eden radyal modellerin yanıltıcı bulgular ortaya koyduğu gözlemlenmiştir. Ayrıca sağlık hizmetleri etkinliğinin, yüksek gelişmişlik düzeyinde bulunan illerin yer aldığı gruptan düşük gelişmişlik düzeyinde bulunan illerin yer aldığı gruba doğru gidildikçe azaldığı görülmüştür. Bunun yanı sıra, Türkiye’deki sağlık hizmetlerinin üretim teknolojisinde gerileme yaşandığı saptanmıştır.
The country’s close relationship between health indicators and development levels, the high share of health spending in non-profit domestic revenue and the increased demand for health services have led to concerns about the effective use of resources. In order to establish an effective functioning health system, which is the main objective of governments, the effectiveness and efficiency of health services must be systematically evaluated and improved by identifying areas that do not work effectively. In this study, where health services in Turkey are addressed at the macro level, the measurement of the effectiveness of health services in the province for 2016 and the change in the efficiency of health services in the province for the period 2012-2016 aimed at comparing the findings obtained from the models of radial and non-radial data coverage analysis (VZA) and Malmquist efficiency index (MVE). The provinces in Turkey have been divided into three groups according to the levels of socio-economic development and the analysis has been carried out separately for each group. Although the analysis found that there was a high and meaningful relationship between the radar and non-radar models and that the methods were monotonously connected, the radar models ignoring the variables were observed to show misleading findings. In addition, the activity of health care has been reduced as it goes towards the group where the provinces are at a high level of development from the group where the provinces are at a low level of development are included. In addition, it has been found that healthcare services in Turkey have a decline in production technology.
The close relationship between health indicators and development levels of countries, the high share of health expenditures in gross domestic product and the increasing demand for health services have given rise to concerns about the efficient use of resources. In order to establish an efficient health system, which is one of the main objectives of governments, it is necessary to evaluate the efficiency and productivity of health services systematically, to identify areas that do not work efficiently and to make improvements. In the study, health services in Turkey are addressed at the macro level. It is aimed to measure the efficiency of health services on provincial basis for the year 2016, to determine the productivity change in health services of provinces for the period of 2012-2016, and to compare the findings obtained from radial and non-radial Data Envelopment Analysis (DEA) and Malmquist Productivity Index (MPI) models. The provinces in Turkey are divided into three groups according to their socio-economic development levels, and all analyses are performed separately for each group. According to the findings, radial and non-radial models are found to be highly correlated and monotonically related, but it is observed that radial models that ignore slack variables produce misleading results. In addition, the health services efficiency decreases from the group of provinces at the highest level of development to the group of provinces at the lowest level of development. Besides, it is determined that health services in Turkey experience a decline in the production technology.
