ÖRGÜTSEL SİNİZM VE ÖRGÜTSEL SONUÇLARININ SAĞLIK ÇALIŞANLARI ÜZERİNE ETKİSİ: SİSTEMATİK BİR DERLEME

Abstract:

Örgütsel sinizm, bireyin içinde bulunduğu kuruma karşı olumsuz bir duygu, tutum ve davranış geliştirmesidir. Bu çalışmanın amacı, örgütsel sinizm ve örgütsel sonuçları (iş tatmini, performans, bağlılık, tükenmişlik, hasta güvenliği kültürü, örgütsel sessizlik örgütsel vatandaşlık) arasındaki ilişkiyi inceleyerek sağlık sektörü çalışanlarına hizmet kalitesini iyileştirme bağlamında önerilerde bulunmaktır. Kavramın, sağlık çalışanları üzerindeki etkisinin değerlendirilmesi açısından ilk sistematik derleme olması ve sinizmin etkilediği tüm örgütsel değişkenlerin bütüncül bir yaklaşımla ele alınmasının bu çalışmanın önemini arttırdığına inanılmaktadır. Çalışma kapsamında, 2011 – 2019 yılları arasında Türkiye’de örgütsel sinizm konusunda yapılmış olan 240 çalışma sistematik olarak incelenmiştir. “Sinizm” “Örgütsel Sinizm”, “Sağlık Çalışanları”, “Hastane” ve “Hasta güvenliği” anahtar sözcükleri Google Akademik arama motoru, DergiPark ve Yök Tez veri tabanlarında taranmış toplam 15 yayın çalışma kapsamına alınmıştır (n = 15). Araştırma sonucunda Türkiye’de sağlık alanında örgütsel sinizm ile ilgili çalışmaların en fazla İstanbul ve Ankara’da yapıldığı ve 2018 yılına kadar doktora tezi yapılmadığı belirlenmiştir. Çalışma sonucunda örgütsel sessizlik kavramının sağlık sektöründe diğer sektörlere oranla daha az çalışıldığı belirlenmiştir. Kavramın örgütsel sessizlik, tükenmişlik ve örgütsel adalet ile ilişkisinin incelendiği çalışmalar yoğunluk kazanırken hasta güvenliği ve kalite ile ilişkisinin değerlendirildiği çalışmaların yetersiz olduğu tespit edilmiştir.

Organizational sinism is the development of negative feelings, attitudes and behaviors towards the institution within the individual. The aim of this study is to study the relationship between organizational sinism and organizational results (work satisfaction, performance, commitment, exhaust, patient safety culture, organizational silence, organizational citizenship), and to present recommendations to healthcare workers in the context of improving the quality of service. It is believed that the concept is the first systematic aggregation in terms of the assessment of its impact on health workers and that the integrated approach of all the organizational variables influenced by sinism increases the importance of this study. In the framework of the study, 240 studies on organizational sinism in Turkey between 2011 and 2019 have been systematically studied. The keywords "Synism", "Organized Synism", "Health Workers", "Hospital" and "Patient Safety" are scanned in the Google Academic Search Engine, Magazine Park and Yök Tez databases; a total of 15 publications are included in the study scope (n = 15). According to the research, the studies on organizational sinism in the field of health in Turkey have been conducted in Istanbul and Ankara and the doctoral thesis has not been conducted until 2018. The study found that the concept of organizational silence was less employed in the health sector than in other sectors. The studies that study the relationship of the concept with organizational silence, exhaust and organizational justice gain intensity; the studies that assess the relationship with patient safety and quality have been found to be insufficient.

Organizational cynicism is the development of a negative emotion, attitude and behavior towards the institution that the individual is in. The aim of this study is by examining the relationship between organizational cynicism and organizational results (job satisfaction, performance, commitment, burnout, culture of patient safety, organizational silence organizational citizenship); to make recommendations to healthcare professionals in terms of improving service quality. It is believed that this concept is the first systematic review in terms of evaluating the impact of the concept on health workers, and the handling of all organizational variables influenced by cynicism with a holistic approach increases the importance of this study. In the study, 2011 - 2019 on the subject of organizational cynicism among the 240 studies examined systematically in Turkey. The keywords “cynicism”, “organizational cynicism”, “healthcare professionals”, “hospital” and “patient safety” have been searched in Google Scholar search engine, DergiPark and Yök Thesis databases; a total of 15 publications were included in the study (n = 15). Istanbul and up to a maximum of dissertation in 2018 in Ankara made and the work on organizational cynicism in the health sector in Turkey was determined to make research results. In the results of working; It has been determined that the concept of organizational silence is less in the health sector compared to other sectors. While the studies examining the relationship between the concept and organizational silence, burnout and organizational justice are gaining intensity; It was found that studies evaluating patient safety and quality with inadequacy were inadequate.
