Amaç: Bu çalışmada ameliyathane dışı anestezi deneyimlerimizi ve sonuçlarını tartışmayı amaçladık. Gereç ve Yöntemler: Mayıs 2017-Aralık 2017 tarihleri arasında tanı ve tedavi amacıyla ameliyathane dışında sedo-analjezi uygulaması yapılan 18 yaş üstü olguların anestezi kayıtları retrospektif olarak tarandı. Kayıtlar; demografik veriler, uygulanan işlem, işlemin süresi, sedasyon derecesi, kullanılan ilaçlar, gelişen minör ve major komplikasyonlar, kronik obstrüktif akciğer hastalığı öyküsü ve yoğunbakım ihtiyacı açısından incelendi. Komplikasyonlar ile demografik veriler ve kategorik değişkenler arasındaki ilişkiler analiz edildi. Bulgular: Toplam 2562 hastaya sedo-analjezi uygulandı. Bu olguların 1428 (%55.7)’i kadın, 1134 (%44.3)’ü erkek idi. Yaş ortalaması 53.08±16.55 idi. Olguların 268 (%10.5)’i ASA I, 1683 (%65.7)’ü ASA II, 598 (%23.3)’i ASA III, 13 (%0.5)’ü ASA IV idi. 519 (%20.3) hastaya minimal sedasyon, 1541 (%60.1) hastaya orta derecede sedasyon, 502 (%19.6) hastaya derin sedasyon uygulandı. En uzun işlem süresi endoskopik retrograd kolanjiopankreatografide 28.7±16.3 dk, en kısa işlem süresi üst gastrointestinal endoskopide 10.3±3.1 dk olarak tespit edildi. En fazla uygulanan işlem %31 ile kolonoskopi idi. En çok kullanılan ilaç kombinasyonu 1231(%48) hastaya uygulanan midazolam+propofol+fentanil kombinasyonu idi. 148 (%5.76) hastada minör komplikasyon, 10 (%0.37) hastada major komplikasyon gelişti. Toplam 15 (%0.58) hasta prosedür sonrası yoğunbakıma devredildi. Desatürasyon; ASA III ve üzeri hastalarda,15 dakikadan uzun süren işlemlerde, 65 yaş üstü hastalarda ve kronik obstrüktif akciğer hastalığı varlığında istatistiksel olarak anlamlı oranda yüksekti. İşlem sonrası yoğunbakım ihtiyacı da bu hasta gruplarında anlamlı oranda yüksekti. Sonuç: Ameliyathane dışı anestezi uygulamalarının sıklığı giderek artmaktadır. Bu uygulamalar taşıdığı riskler açısından ameliyathanedeki anestezi uygulamalarıyla benzerdir. Hasta güvenliğinin artırılması ve komplikasyonların azaltılması için işlem öncesi ayrıntılı bir değerlendirme yapılması, uygun fiziksel şartlarda doğru bir monitörize anestezi bakımı sağlanması ve ekipler arasında sağlıklı bir iletişim kurulması önemlidir.
Purpose: In this study, we aimed at discussing our experience and outcomes of non-chirurgical anesthesia. Tools and Methods: Anesthesial records of 18 years of age and over who were applied sedo-analysis outside the hospital for the purpose of diagnosis and treatment between May 2017 and December 2017 were scaned retrospectively. The records were studied in terms of demographic data, the process applied, the duration of the process, the degree of sedation, the drugs used, developing minor and major complications, the history of chronic obstructive lung disease and the need for intensive care. Relationships between complications and demographic data and category variables were analyzed. In total, 2562 patients were subjected to sedo-analysis. Of these facts, 1428 (55.7%) were women, and 1134 (44.3%) were men. The average age was 53.08±16.55. Of the cases, 268 (10.5%) were ASA I, 1683 (65.7%) ASA II, 598 (23.3%) ASA III, 13 (0.5%) ASA IV. 519 (%20.3) patients received minimal sedation, 1541 (%60.1) patients received moderate sedation, 502 (%19.6) patients received deep sedation. The longest processing time was detected in endoscopic retrograde colangiopankreatography 28.7±16.3 minutes, the shortest processing time was detected in the upper gastrointestinal endoscope 10.3±3.1 minutes. The most used procedure was colonoscopy with 31%. The most commonly used drug combination was the midazolam+propofol+fentanil combination that was administered to 1231(48%) patients. 148 (5.76) patients developed minor complications, 10 (0.37) patients developed major complications. A total of 15 (0.58) patients were transferred to intensity after the procedure. Desaturation increased statistically significantly in patients with ASA III and above, in patients over the age of 15 minutes, in patients over the age of 65 and in the presence of chronic obstructive lung disease. The need for intensive care after the procedure was also significantly high in these patients groups. The frequency of non-operative anesthesia is increasing. These practices are similar to the use of anesthesia in the surgery in terms of the risks it carries. It is important to make a detailed pre-operative assessment to improve patient safety and reduce complications, to ensure proper monitoring and anesthesia care in appropriate physical conditions, and to establish a healthy communication between the teams.
