Our objective is to search the HMWCK 34bE12 immunoreactivity in high grade PIN lesions, atrophy, adenosis, basal cell hyperplasia (BCH) and normal prostatic tissue. Cases were immunostained by 34bE12. We determined the percentage, type (continuous, focally interrupted and patchy) and the density (weak, intermediate and strong) of staining. The percentage of staining was significantly low in adenosis cases. Normal or atrophic prostate glands sometimes didn’t show positive staining. The staining properties of BCH group (% 98 continuous and strong in density) were significantly different from others. High grade PIN lesions also sometimes showed focally interrupted or even patchy staining. Nonreactive glands must be utilized according to their location in the lesion and compared to the cytologically similar ones. It is not recommended to diagnose a lesion as cancer by showing nonreactivity in a small amount of suspicious glands. The probability of false negative staining must be always in mind.
