Amaç: Hastane enfeksiyonlarının en fazla görüldüğü yerler yoğun bakım üniteleridir. Hastane kaynaklı üriner sistem enfeksiyonu gelişiminin %95’den fazlası üriner kateter kullanımına bağlıdır. Nozokomiyal üriner sistem enfeksiyonları en sık görülen hastane enfeksiyonlarından biridir, morbidite ve mortalitede oluşturduğu artışın yanında hastanede kalış süresini uzatarak tedavi maliyetinin artmasına neden olur. Bu çalışmada yoğun bakımda kateter ilişkili üriner sistem enfeksiyon hızının belirlenmesi, sürveyans verileri elde etmek ve yorumlamak amaçlandı.Gereç ve Yöntemler: Hastanemiz yoğun bakım ünitesinde 01.01.2017-01.12.2018 tarihleri arasında yatan ve üriner katetere bağlı gelişen nozokomiyal üriner sistem enfeksiyonu olan hastaların elektronik hasta dökümanları retrospektif olarak incelendi.Bulgular: Hastanemiz yoğun bakım ünitesinde takip edilen hastalarda üriner kateter ile ilişkili üriner sistem enfeksiyon hızı 11,9 olarak bulundu. Sıklık sırasına göre etkenler Escherichia coli, Pseudomonas spp., Enterococcus spp., Klebsiella spp., Acinetobacter spp., Candida spp., Enterobacter spp. olarak saptandı. E. coli’de %42, genişlemiş spektrumlu beta laktamaz pozitifliği, Enterococcus spp. etkenlerinde %33 penisilin, Pseudomonas spp. etkenlerinde %34,5 karbapenem ve Acinetobacter spp. etkenlerinde %56 karbapenem ve tamamında tigesiklin direnci olduğu saptandı.Sonuç: Yoğun bakım ünitelerindeki hastalardan izole edilen mikroorganizmalar ve onların direnç profilleri her hastanede farklıdır. Etkili bir enfeksiyon kontrol programı oluşturup yürütebilmek için her hastanenin kendi sürveyans kayıtlarını oluşturması ve direnç profilini bilmesi gereklidir
Goal: The places where hospitalized infections are most common are intensive care units. More than 95% of the development of urine system infection from the hospital is dependent on the use of urine catheter. Nozocomial urinary system infections are one of the most common hospitalized infections, along with the increase in morbidity and mortality, which leads to an increase in treatment costs by prolonging the stay in the hospital. This study was aimed at determining and interpreting the rate of infection of the urine system associated with catheter in intensive care.The purpose and methods: our hospital in the intensive care unit 01.01.2017-01.12. Electronic patient documents of patients with nozo-comal urine system infection that develops between the dates of 2018 and urine catheters have been examined retrospectively.The findings: In our hospital intensive care unit followed patients with urine catheters related urine system infection rate was 11.9. According to frequency orders, the factors are Escherichia coli, Pseudomonas spp., Enterococcus spp., Klebsiella spp., Acinetobacter spp., Candida spp., Enterobacter spp. It was identified. In E. coli, 42% of extended spectrum beta lactamic positivity, Enterococcus spp. 33% penicillin, Pseudomonas spp. 34,5% carbapenem and Acinetobacter spp. It was found that 56% of the factors contained carbapenem and tigesiclin resistance. The result: the isolated microorganisms and their resistance profiles from patients in intensive care units are different in each hospital. To create and run an effective infection control program, it is necessary for each hospital to create their own surveillance records and know the resistance profile.
