Kaynakların kıtlığı ekonominin bir gerçeği olarak kaynak dağıtımı kararlarının verilmesi yanında aynı zamanda mevcut kaynakların etkin kullanımını da gerektirmektedir. Bu yönüyle Türkiye’de son yıllarda ağız ve diş sağlığı hizmeti sunan kurumların mevcut talebi karşılamada yetersiz olması bu kurumların niceliğini artırmaya yönelik kaynakların tahsisi ile sonuçlanmıştır. Bu bağlamda uzun vadede yeni kaynak tahsisi ile sağlanacak faydanın yanı sıra, kısa vadede mevcut kaynakların ne ölçüde etkin kullanıldığı değerlendirilerek fayda sağlamakta mümkündür. Bu bakışla araştırmada Ankara, İstanbul ve İzmir illerinde faaliyet gösteren Sağlık Bakanlığı’na bağlı ağız ve diş sağlığı merkezleri (ADSM) ile ağız ve diş sağlığı hastanelerinin (ADSH) etkinlikleri değerlendirilmiştir. Belirtilen sağlık tesislerinin etkinlik değerlendirilmesinde parametrik olmayan bir yöntem olan Veri Zarflama Analizi (VZA) kullanılmıştır. Analizde girdi değişkeni olarak diş üniti sayısı, toplam diş hekimi sayısı ve diğer sağlık hizmetleri personeli sayısı çıktı değişkeni olarak poliklinik (muayene) sayısı, diş çekimi sayısı, cerrahi müdahale sayısı, kanal (endodontik) ve dolgu (konservatif) tedavi sayısı ve toplam protez sayısı belirlenmiştir. Ölçeğe göre sabit getiri ve ölçeğe göre değişken getiri varsayımları altında yapılan analiz sonucunda 36 sağlık tesisinden 23’ünün (%63,9) teknik etkin, 13’ünün (%36,1) toplam etkin ve 14’ünün (38,9) ölçek etkin olduğu saptanmıştır. Analizde sağlık tesislerinin özellikle teknik etkinlik skoru ortalaması (0,937) oldukça yüksek bulunmakla birlikte, teknik etkin olmadığı saptanan sağlık tesislerinin etkin olabilmesi için referans grubu belirlenerek etkinlik hedefleri oluşturulmuştur.
As a fact of the economy, scarcity of resources requires resource allocation decisions as well as the effective use of available resources. In this respect in recent years, oral and dental health services is insufficient to meet the current demands of the offering institution in Turkey has resulted in the allocation of resources to increase the quantity of these institutions. In this context, it is possible to provide benefits by evaluating to what extent the available resources are effectively used in the short term, as well as the benefit to be provided through the allocation of new resources in the long term. From this point of view, the effectiveness of oral and dental health centers (ADSM) and oral and dental health hospitals (ADSH) affiliated to the Ministry of Health in Ankara, Istanbul and Izmir provinces were evaluated. In this study Data Envelopment Analysis (DEA), a non-parametric method, was used to evaluate the efficiency of healthcare facilities. In this analysis, the number of dental units, the number of total dentists and the number of other health care personnel were chosen as input variables and the number of outpatients (examination), the number of tooth extractions, the number of surgical operations, the total number of channels (endodontic) and filler (conservative) treatments and the number of total prosthesis were chosen as output variables. As a result of the analysis made under assumptions of constant return to scale and variable return to scale, of 36 health facilities, 23 (63.9%) were found technical efficient, 13 (36.1%) were total efficient and 14 (38.9) were scale efficient. In the analysis, the average technical effectiveness score of the health facilities (0.937) was found to be quite high and reference groups were determined and effectiveness targets were established in order to ensure inefficient health facilities were effective.
