Ductal carcinoma of the prostate is a rare histologic subtype of prostate carcinoma. It represents 0.2% of all prostate cancers and is associated with a poor prognosis. Our case , 78 years old man, applied our clinic with complaint of of hematuria. Papillary urethral mass was shown on verru montanum by endoscopy. We performed transurethral resection. Diagnosis of prostate ductal adenocarsinoma was diagnosed by the result of histopatologic and immunohistochemical examination. The patient was treated by hormonotherapy and radiotherapy and received a positive response.The ductal adenocarcinoma of prostate is a rarely seen tumor. The tumors have poor prognoses and may require aggressive treatment. Therefore, the early diagnosis and treatment of ductal adenocarcinoma is very important. The ductal adenocarcinoma should be considered in the differential diagnosis of the prostate carcinomas
