Amaç: Tiroidektomi sonrası geçici hipokalsemi en sık görülen komplikasyondur ve oluştuğunda kolay tedavi edilir. Geçici hipokalsemi ile ilişkili esas problem hastanede kalış süresini uzatmasıdır. Bu çalışmanın amacı tiroid cerrahisi uygulanan hasta grubunda postoperatif geçici hipokalsemi için risk faktörlerini belirlemekti. Gereç ve Yöntem: Ocak 2012 - Aralık 2013 tarihleri arasında total tiroidektomi uygulanan 177 hastanın verileri retrospektif olarak değerlendirildi. Hipokalsemi total serum kalsiyum düzeyinin 8 mg/dl altında olması olarak tanımlandı. Geçici hipokalsemi total tiroidektomiyi takiben 6 ayda hipokalseminin iyileşmesi olarak tanımlandı. Geçici hipokalsemi için risk faktörleri olarak cinsiyet, preoperatif D vitamini eksikliği, reküren hastalık için cerrahi girişim, hipertiroidi varlığı, görülen ve korunan paratiroid bez sayısı, paratiroid bez ekimi yapılması, patolojik spesmende çıkarılan paratiroid bezi varlığı değerlendirildi. İstatistik değerlendirmede ‘Nominal Lojistik Regresyon’ analizi, ‘Ki-kare’ testi ve ‘Fisher’in Kesinlik’ testi kullanıldı. Bulgular: Çalışmadaki 177 hastanın (150K, 27E) 37’sinde (%20.9) geçici hipokalsemi gelişti. Nominal regresyon analizinde sadece patolojik spesmende çıkarılan paratiroid bezi varlığı (p=0.025) geçici hipokalsemi için bağımsız değişken faktör olarak belirlendi. Sonuç: Patolojik spesmende paratiroid bezi varlığı yüksek oranda geçici hipokalsemiden sorumludur. Tiroidektomi esnasında cerrahi spesmenin intraoperatif dikkatli incelenmesi uygunsuz paratiroidektomi insidansını azaltabilir.
Purpose: Temporary hypocalcemia after thyroidectomy is the most common complication and is easy to treat when it occurs. The main problem associated with temporary hypocalcemia is the extension of the stay in the hospital. The aim of this study was to identify the risk factors for postoperative temporary hypokalcemia in the group of patients under thyroid surgery. The data of 177 patients with total thyroidectomy between January 2012 and December 2013 were retrospective. Hypokalcemia was defined as a total serum calcium level below 8 mg/dl. The temporary hypokalcemia was defined as a healing of hypokalcemia within 6 months after total thyroidectomy. Risk factors for temporary hypokalcemia are gender, preoperative vitamin D deficiency, surgery for recurrent disease, hyperthyroid presence, the number of parathyroid glands observed and preserved, parathyroid glands plantation, the presence of parathyroid glands extracted from the pathological sperm. The statistical assessment used the 'Nominal Logistic Regression' analysis, the 'Ki-Care' test and the 'Fisher's Precision' test. Results: In 177 patients (150K, 27E) in the study, 37 (20.9%) developed temporary hypokalcemia. In the nominal regression analysis, the presence of parathyroid glands (p=0.025) extracted only in the pathological spectrum was determined as an independent variable factor for temporary hypokalcemia. The result: the presence of parathyroid glands in the pathological spess is responsible for a high degree of temporary hypokalcemia. The careful intraoperative examination of the surgical sputum during thyroidectomy may reduce the incidence of inappropriate parathyroidectomy.
