Purpura Fulminans ekstremitelerde cilt ve yumuşak dokunun hemorajik nekrozu ile seyreden, mortalite ve morbiditesi yüksek bir hastalıktır. Eklem bölgelerindeki kontraktür gelişimi, ekstremitelerde fonksiyon kaybı ile sonuçlanmaktadır. Cilt defektlerinin onarımında gümüş içerikli yara bakım ürünlerinin kullanımı, asellüler dermal matriks ve kısmi kalınlıkta deri grefti uygulamaları güncel plastik cerrahi yaklaşımlardandır. %55 vücut yüzey alanı nekrozlu 12 yaşındaki purpura fulminans tanısı alan pediatrik olgumuzda, seri debridmanlar ve gümüş içerikli yara örtüleri ile takip sonrası ekstremite eklem fonksiyonunu korumaya yönelik onarımda deri eşdeğeri MatriDerm ilk kez uygulandı. Hastanın alt ekstremite eklem hareketlerinde ekstansiyon ve fleksiyonda ROM range of motion ’da kısıtlılık saptanmadı
Purpura Fulminans is a disease with high mortality and morbidity, seen with hemorrhagic necrosis of the skin and soft tissue in the limbs. The development of contracture in the joint areas results in loss of function in the limbs. The use of wound care products containing silver in the repair of skin defects, the primary dermal matrix and the partial thickness of the skin injection applications are among the current plastic surgical approaches. In our pediatric phenomenon with a 12-year-old diagnosed purpura fulminans with 55% body surface area necrosis, the skin equivalent MatriDerm was first applied in the repair to maintain the joint function with serial debridments and silver-containing wound coverings. The patient's lower end joint movements in extension and flektion in the ROM range of motion have not been detected.
