Abstract:

Purpose: In cases of plantar phasitis that cannot be responded to conservative therapy, it is intended to compare the effectiveness of rESWT (radial extrakorporeal shock wave therapy) applied by two different treatment protocols. Tools and Methods: Between 2010 and 2017, 319 patients applied rESWT in a single center and met our inclusion criteria were evaluated. 181 patients with 1 week interval 1.5 bar, 10Hz, 1500 shock, a total of 5 sessions(group 1) and 138 patients with 1 week interval 3 bar, 10Hz, 2000 shock, 5 sessions(group 2) rESWT were applied. Clinical and functional results were assessed immediately before treatment, in the 6th week and 6th month of treatment with Visual Analog Scale (VAS) and American Orthopedic Foot and Foot Compound Score (AOFAS). Results: The average VAS score dropped from 8.73 (±1.10) to 3.20 (±2.09) (p=0,0001) in group 1 and from 8.68 (±1.17) to 2.61(±1.68) (p=0,0001) in group 2. The average AOFAS score increased from 55.85 (± 11.73) to 88.38 (± 8.23) (p=0,0001) in group 1 and 58.74 (± 12.74) to 88.47 (± 8.10) (p=0,0001) in group 2. 6th and 6th week of Group 1 and 2. While statistically significant differences in VAS scores are not detected (p=0.451), group 1 is pre-treatment-6. 1 week and 6 weeks before treatment. Month VAS values changes were found significantly lower than group 2 (p=0,006, p=0,019). Pre-treatment of group 1 and group 2 is 6. 6th and 6th week. The AOFAS score changes are not statistically significant (p=0,094, p=0,174), while the group 1's pre-treatment-6. Weekly AOFAS score change is significantly lower than group 2 (p=0,029). Result: Although both treatment protocols also contribute to the reduction of pain and function loss, it can be said that the 5 session rESWT is superior to the 5 session rESWT applied with a total of 2000 shocks at 1 bar air pressure and 10Hz frequency.
