Abstract:

Objectives:To determine the efficacy of gender, stone size, stone localization and degree of hydronephrosis in semirigid ureteroscopy success for ureteral stones.Materials and Methods:We retrospectively evaluated 244 patients with ureteral stones trated with semirigid ureteroscopy between February 2003 and August 2011. Patient was seperated two groups that the stone size was larger and less than 15 mm. Stone localization was classified lower, middle and upper ureter. Hydronephrosis degree was classified none, mild, modarete, severe and serious. Stone size, localization and hydronephrosis degree were evaluated from ultrasonography and computerized tomography. Results: The mean age of the patients was 38.3 years . Of the 244 patients 98 %40.1 were female and 146 %59.9 were male. Of the stones 108 %44.3 were right side,124 %50.8 left side and 12 %4.9 bilateral. Of the stones 144 %59 were localized lower ureter, 30 %12.3 middle ureter and 70 %28.7 upper ureter. Of the patients 36 %14.8 had no hydronephrosis; 66 %31.7 patiens had mild, 104 %50 modarete, 32 %15.4 severe and 6 %2.9 serious hydronephrosis. The mean stone diameter was 12.4 mm. Stone free rate was % 84.1. There was no efficacy of gender on the success rate p=0.9 . The success rate of lower, middle and upper ureter stones were % 93.8, % 83.3 and % 64.3 p=0.04 . According to hydronephrosis degree, the success rates were %64.3 olarak bulundu p=0.04 . Hidronefrozu olmayan hastalarda %91.7 başarı oranı sağlandı; hidronefroz derecesine göre ise sırasıyla %89.4, %83.7, %68.8 ve %66.7 olarak bulundu p=0.027 . Taş büyüklüğüne göre ise 15 mm’nin altındaki taşlarda %89.2 15 mm’den büyük taşlara oranla %64.6 istatistiksel olarak anlamlı bir başarı oranı sağlanmıştır p=0.035 . 244 hastanın sadece 1’inde % 0.4 üreteral perforasyon nedeniyle açık cerrahiye geçilmiştir.Sonuç: Üreter taşı tedavisinde semirijid üreterorenoskopinin başarısını artıran faktörler 15 mm’den küçük taşlar, alt-orta üreter taşlar ve hidronefrozu olmayan veya düşük derecelerde hidronefro

Stone free rate was 84.1%. There was no efficacy of gender on the success rate p=0.9 . The success rate of lower, middle and upper ureter stones were 93.8% and 83.3% and 64.3% p=0.04 . According to hydronephrosis degree, the success rates were 64.3 % found as p=0.04 . In patients with no hydronephrosis, 91.7 percent of success was achieved; according to hydronephrosis, 89.4 percent, 83.7 percent, 68.8 percent and 66.7 percent were found p=0.027 respectively. According to stone size, stones below 15 mm have a statistically significant success rate of 64.6 % compared to stones greater than 89.2 % 15 mm p=0.035 . Only 1 in 244 patients had 0.4 per cent of the outer perforation.Result: Factors that increase the success of semirijid ureterorenoscopy in the treatment of producer stone are stones of 15 mm, sub-mediate producer stones and hydronefrozy-free or low levels of hydronefrozy.
