Amaç: Bu çalışmada benign ve malign tiroid nodüllerindeki ultrasonografi bulgularının araştırılması amaçlandı. Gereç ve Yöntem: Haziran 2008-Haziran 2009 tarihleri arasında Rize Eğitim ve Araştırma Hastanesi Radyoloji bölümünde ultrasonografi eşliğinde yapılan ve sonuçları yeterli bulunan toplam 71 (14 erkek, 57 kadın; ortalama yaş: 47±12) olguya ait ince iğne aspirasyon biyopsisi sonuçları ile tiroid ultrasonografisi bulguları karşılaştırıldı. Ultrasonografide nodül sayısı, ekojenitesi (hipoekoik, izoekoik, hiperekoik, karışık ekojenitede), şekli (lobüle, lobüle olmayan), sınır özellikleri (düzgün, belirsiz), iç yapısı (homojen, heterojen) ve nodül boyutları ile kalsifikasyon (mikro, makro, çeper), kistik alan ve halo içerip içermedikleri değerlendirildi. Bulgular: Tiroid ince iğne aspirasyon biyopsileri sonucunda; 49 (%69) olgu benign, 2 (%2.8) olgu tiroidit, 7 (%9.9) olgu malignite açısından şüpheli, 3 (%4.2) olgu malign ve 10 (%14.1) olgu folliküler neoplazi olarak değerlendirildi. Bu sonuçlar benign (benign ile tiroidit) ve malign-şüpheli (malign, malignite açısından şüpheli ve folliküler neoplazi) olarak sınıflandırıldı. İstatiksel olarak anlamlı (p<0.05) ultrasonografi bulguları malign-şüpheli grupta 11 (%55) olguda hipoekoik yapı (%55 duyarlılık, %78 özgüllük) ve 16 (%80) olguda belirsiz sınır özelliği (%80 duyarlılık, %50 özgüllük) olarak bulundu. Sonuç: Ultrasonografi bulgularından maligniteyi desteklemede en önemlileri hipoekojenite ve belirsiz sınır özellikleri olarak bulunmuş olup ince iğne aspirasyon biyopsisinde nodül seçimi açısından yol gösterici olabileceği düşünülmektedir.
Purpose: This study was aimed at investigating the ultrasound findings in the benign and malign thyroid nodes. Instruments and Methods: between June 2008 and June 2009 in the Rize Education and Research Hospital’s Department of Radiology was performed with ultrasound accompanying and the results were sufficient in a total of 71 (14 men, 57 women; average age: 47±12) with the results of thyroid ultrasound compared with the results of thyroid aspiration biopsy. In ultrasonography, the number of nodes, the ecogenity (hypoecological, isoecological, hyperecological, mixed ecogenity), the shape (lobule, non-lobule), the limit characteristics (right, uncertain), the internal structure (homogeneous, heterogeneous) and the nodes, and the dimensions of the calcification (micro, macro, square), the cystic area and halo were assessed. The findings: Thyroid thin injection aspiration biopsy result; 49 (%69) cases were benign, 2 (%2.8) cases of thyroiditis, 7 (%9.9) cases of malignity suspected, 3 (%4.2) cases of malignity and 10 (%14.1) cases of follicular neoplasm were considered. These results were classified as benign (benign with thyroiditis) and malignly suspicious (malign, malignly suspicious and follicular neoplasm). Statically meaningful (p<0.05) ultrasound findings were found in the malignant group as hypoecological structure (55% sensitivity, 78% specificity) and in the 16% (80%) uncertain limit characteristics (80% sensitivity, 50% specificity). Result: The most important in supporting malignity from ultrasound findings are found as hypoecogenicity and uncertain boundary characteristics, and it is believed that thin injection aspiration biopsy can be a guide in terms of nodular selection.
