Amaç: Araştırmanın amacı, maternal obezitenin anne ve yenidoğan sağlığına etkisini belirlemektir. Gereç ve Yöntem: Vaka-kontrol çalışması olarak planlanan araştırma, Mart 2015-Ağustos 2016 tarihleri arasında, İskenderun Devlet Hastanesi loğusa servisinde yapılmıştır. Çalışmaya alınan kadınlar gebe kalmadan önceki beden kitle indekslerine göre normal kilolu (kontrol grubu n=144) ve obez (vaka grubu n=142) olmak üzere iki gruba ayrılmış ve toplam 286 kadın ile çalışma tamamlanmıştır. Verilerin toplanmasında, Veri Toplama Formu ve Edinburgh Postpartum Depresyon Ölçeği kullanılmıştır. Veriler yüz yüze görüşme yöntemi, hasta dosyası ve doğumdan altı hafta sonra telefon görüşmesi ile toplanmıştır. Verilerin analizinde tanımlayıcı istatistikler, ki-kare ve student t testi kullanılmıştır. Bulgular: Vaka grubu kadınların yaş, gebelik ve yaşayan çocuk sayısı ortalamalarının kontrol grubu kadınlara göre daha yüksek, eğitim düzeylerinin ve doğum haftası ortalamalarının ise daha düşük olduğu saptanmıştır (p<0,05). Vaka grubu kadınlarda, gebelikte idrar yolu enfeksiyonu, anemi, gestasyonel diyabet, psikolojik problemler, preeklampsi/gestasyonel hipertansiyon, doğumda fundal basınç uygulaması, doğum sonu erken ve geç dönem komplikasyonlar kontrol grubu kadınlara göre daha yüksek bulunmuştur (p<0,05). Vaka grubunda doğum haftası ortalamaları ve yenidoğanların Apgar puan ortalamaları daha düşük, yoğun bakıma alınma oranları daha yüksektir (p<0,05). Vaka grubunda bir bebek doğumundan sonra yaşamını yitirmiştir. Sonuç: Çalışma sonuçlarına göre maternal obezitenin gebelik ve doğum sonu dönemde birçok komplikasyonlara yol açtığı, yenidoğan morbiditesinin artmasında da önemli bir risk faktörü olduğu sonucuna varılmıştır. Gebelik planlayan kadınların prekonsepsiyonel dönemden itibaren obezite yönünden taranmaları, obezitesi olan kadınlara eğitim ve danışmanlık yapılması önerilebilir.
Purpose: The purpose of the study is to determine the impact of maternal obesity on the health of the mother and newborn. Tools and Methods: The research planned as a case-control study, was carried out between March 2015-August 2016, at the Alexandrian State Hospital's logusa service. According to the body mass index before pregnancy, the study was divided into two groups: normal weight (control group n=144) and obesity (case group n=142) and the study was completed with a total of 286 women. In the collection of data, data collection form and Edinburgh Postpartum Depression Scale were used. The data was collected by the face-to-face conversation method, the patient’s file and the phone call six weeks after birth. In the analysis of data, identifiable statistics, ki-quare and student t tests were used. Results: The case group found that the average age, pregnancy and number of living children of women were higher than the control group, while the educational levels and the average week of birth were lower (p<0,05). The case group in women, urinary infection during pregnancy, anemia, gestational diabetes, psychological problems, preeclampsia/gestational hypertension, fundal pressure application in childbirth, early and late birth complications control group was higher than in women (p<0,05). In the case group, the birth week average and the Apgar score average of newborns are lower, the intensive care rates are higher (p<0,05). A baby in the group of cases has died after birth. Results: The study finds that maternal obesity causes many complications during pregnancy and late childbirth, and is also an important risk factor in the increase in newborn morbidity. Women planning pregnancy from the preconceptional period can be recommended to be examined in the direction of obesity, education and consultation to women with obesity.
