Amaç: Helikobakter pylori’nin (H.pylori) sırasıyla kronik aktif gastrit, atrofik gastrit, istestinal metaplazi, displazi, gastrik kanser gelişiminde rol oynadığı saptanmıştır. Çalışmamızda endoskopik inceleme yapılan olguların; endoskopik, patolojik ve laboratuvar (Hemogram ve demir değerleri) bulguları açısından değerlendirilmesini amaçladık. Yöntem: 2015-2016 yılları arasında, gastrointestinal sistem kaynaklı şikayetleri nedeniyle endoskopik inceleme ve laboratuvar tetkiki istenen 359 olgu çalışmaya dahil edildi. Bulgular: Gruplar arası değerlendirmede [Grup I (H.pylori (+)) – Grup II (H.pylori (-))]; yaş Grup I’de 43±13.6, Grup II’de 47.7±16.7 yıl saptandı ve istatistiksel olarak anlamlı saptandı (p=0.004). Demir bağlama kapasitesi Grup I’de 319.3±74.9 μg/dl, Grup II’de 293.7±82.4 μg/dl olarak saptandı (p=0.002). Bulbusta ülser varlığı değerlendirildiğinde; Grup I’de 49, Grup II’de 32 olgu saptandı (p=0.01). Nötrofil infiltrasyonunun Grup I’deki olgularda yüksek olduğu saptandı (p=0.000). H.pylori varlığının korelasyon incelemesinde; nötrofil infiltrasyonu, bulbusta ülser varlığı, demir bağlama kapasitesi ile pozitif korelasyon, yaş ile negatif korelasyon gösterdiği saptanmıştır. Kronik inflamasyon varlığında ise istatistiksel olarak (r=.102, p=0.054) anlamlılık saptanmamış olmasına rağmen pozitif korelasyon açısından dikkat çekicidir. Sonuç: Özellikle genç erişkinlerde patolojik bulgular ile birlikte demir bağlama kapasitesinde artış etiyolojisi tam olarak bilinmeyen anemi tablosunun gelişimi için öncü değerler olarak kullanılabilir. Fakat bunu desteklemek amacıyla yeni çalışmalara ihtiyaç vardır.
Purpose: Helicobacter pylori (H.pylori) has been found to play a role in the development of chronic active gastritis, atrophic gastritis, instinal metaplasy, dysplasia, and gastric cancer. In our study, we aimed at evaluating the facts of endoscopic examination in terms of endoscopic, pathological and laboratory findings (hemogram and iron values). Method: Between 2015-2016 years, 359 facts required for endoscopic examination and laboratory examination due to the complaints caused by the gastrointestinal system were included in the study. Results: In the intergroup assessment [Group I (H.pylori (+)) - Group II (H.pylori (-)]; age in Group I was 43±13.6, age in Group II was 47.7±16.7 years and statistically meaningful (p=0.004). Iron connectivity capacity is 319.3±74.9 μg/dl in Group I, 293.7±82 in Group II. It was determined as 4 μg/dl (p=0.002). When bulbusta was assessed; in Group I, 49, in Group II, 32 incidents were detected (p=0.01). It was found that the neutrophil infiltration was high in the events in Group I (p=0,000). In the correlation study of H.pylori’s presence, it was found that it showed neutrophil infiltration, bulbular ulceration, positive correlation with iron connectivity, negative correlation with age. In the presence of chronic inflammation, the significance is not statistically identified (r=102, p=0.054) but is remarkable in terms of positive correlation. The result: especially in young adults, with pathological findings, the increase in iron bonding capacity etiology can be used as pioneering values for the development of the unknown anemia table. But new work is needed to support it.
