In the event that individuals borrow from lending institutions to be paid within a certain maturity in order to obtain purchasing power or meet their cash needs it is requested by the lending institution to take out insurance against the risk of death or whole permanent disability of the borrower. This type of insurance is called life insurance linked by personal loan. With the provisions of the Turkish Commercial Code, permanent and reciprocal obligations are regulated between the insurer and the policyholder in insurance contracts. In response to the insurer's "lighting obligation", the insured has a "declaration obligation". In this study, the proportion ratios, requested by the Supreme Court in disputes regarding the violation of the obligation of declaration in this type of insurance, are modeled with actuarial methods by using Turkish data for the first four disease groups (cancer, circulatory system, respiratory system and nervous system) that cause death by using Turkish Statistical Institute Cause of Death Statistics.
