Karpal Tünel Sendromu: 92 Olgunun Geriye Dönük Değerlendirilmesi

Karpal tünel sendromu (KTS), median sinirin el bileği düzeyinde sıkışmasına bağlı olarak gelişen kompresyon nöropatisidir. Bu klinik çalışmada, KTS tanısı ile tedavi edilen 92 olgu ile ilgili deneyimlerimiz sunulmaktadır. Olgular, yaş, cinsiyet, hazırlayıcı unsurlar, fizik muayene bulguları, uygulanan cerrahi tedavi yöntemleri ve komplikasyonlar dikkate alınarak incelendi. Olguların yaş ortalaması 49 olup, 88'i kadın ve 4'ü erkek idi. En sık karşılaşılan hazırlayıcı unsur, ellerin yoğun strese maruz kalması idi. Noktürnal parastezi,sık gözlenen bulgu idi. Tinel bulgusu ve falen testinin duyarlılıkları sırasıyla %62 ve %52 olarak bulundu. Tüm olgularda açık cerrahi dekompresyon uygulandı. Olguların 60'ında klasik, 58'inde ise kısa dikey kesi tercih edildi. Operasyon sonrası erken dönemde dikiş hattında ağrı ve el bileğinde hareket kısıtlılığı, klasik dikey kesi ile açık cerrahi dekompresyon uygulanan olgularda daha sık görüldüğü gözlendi. Sonuçta, kısa dikey kesi ile daha tatminkar sonuçlar elde edildiği ve bu geriye dönük çalışmadan elde edilen sonuçların batı kaynaklarında ki verilerle korelasyon gösterdiği saptandı.

Carpal Tunnel Syndrome (KTS) is a compression neuropathy that develops due to the compression of the median nerve at the handheld level. In this clinical study, we present our experiences with 92 facts treated with the diagnosis of KTS. Facts, age, gender, preparatory elements, physical examination findings, applied surgical therapy methods and complications were studied taking into account. The average age of the cases was 49 and 88 were women and 4 were men. The most common preparatory element was that your hands were exposed to intense stress. Parasthesia was a frequently observed discovery. The sensitivity of the tinel find and the false test were found at 62% and 52% respectively. In all cases, open surgical decompression was applied. In 60 of the cases, the classic was preferred, and in 58 the short vertical cut was preferred. In the early postoperative period, pain and limitation of movement in the wrist line, classic vertical cutting and open surgical decompression were observed more frequently in the cases applied. After all, it was found that the short vertical cutting achieved more satisfactory results and that the results obtained from this backward work were correlated with the data in the Western sources.
