Oral Skuamöz Hücreli Karsinom Olgularının Beş Yıllık Literatür İncelemesi

Ağız kanserlerinin büyük bir kısmını oluşturan oral skuamöz hücreli karsinom (SHK) kanser morbidite ve mortalitesinin önemli bir nedenidir ve ağız içinde çeşitli bölgelerde gözlenebilir. Bu çalışmanın amacı güncel literatürü değerlendirerek oral SHK`nın klinik ve radyografik özelliklerini, tedavi yöntemlerini bildirmektir.Pubmed veri tabanında oral SHK`lara ait son 5 yıl içerisinde olgu sunumu olarak yayınlanmış makaleler araştırıldı. Vakalarla ilgili yetersiz bilgi bulunan makaleler inceleme dışı bırakıldı. Hastaların yaşı, cinsiyeti, tümörün bulunduğu bölge, klinik ve radyografik bulgular, TNM evresi ile tedavi şekilleri ve prognoz analiz edildi.68 makaleye ait 78 hastanın bilgileri (36 kadın, 42 erkek) değerlendirildi. SHK, oral kavitede sırasıyla en sık gingiva (% 33,3), dil (% 23,1), mandibular kemik (% 21,8) ve bukkal mukozada (% 15,4) gözlendi. Hastalarda en sık rastlanan bulgu ülserasyon (% 42,3), en sık rastlanan belirti ise ağrıydı (% 41). Olguların TNM evreleri değerlendirildiğinde çoğunlukla 3. (% 34,5) ve 4. (% 36,4) evrede teşhis koyulduğu belirlendi.Çalışmamızda oral SHK olgularının çoğunun geç evrede teşhis edildiği belirlendi. Ağız kanserlerinin prognozunun kötü olması nedeniyle, erken teşhis hastanın tedavisi ve yaşam kalitesi açısından büyük önem taşımaktadır. Bu nedenle toplumun ağız kanseri ve risk faktörleri açısından bilinçlendirilmesi ve rutin oral muayenelerinin yapılması SHK`nın erken teşhisi açısından önemlidir.

Squamous cell carcinoma (SCC) that constitutes a large part of oral cancer is an important cause of cancer morbidity and mortality, and can be seen in various oral sites. The aim of this study is to report clinical and radiographic features, treatment modalities of oral SCC by evaluating current literature.Articles published as case reports in Pubmed database in last 5 years of oral SCC have been searched. Articles with insufficient information about cases were excluded. Age and gender of patients, region of tumor, clinical and radiographic findings, TNM stage, treatment methods and prognosis were analyzed.Information of 78 patients (36 females, 42 males) from 68 articles were evaluated. The most detected tumor regions in oral cavity were gingiva (33.3%), tongue (23. 1%), mandibular bone (21.8%) and buccal mucosa (15.4%) respectively. The most common sign in patients was ulceration (42.3%), and the most common symptom was pain (41%). Stage 3 (34.5%) and Stage 4 (36.4%) TNM stages were mostly observed.In our study, it was determined that the majority of cases of oral SCC were diagnosed in late stages. Due to the poor prognosis of oral cancer, early diagnosis has a great importance for the treatment and quality of life of the patient. For this reason, raising awareness about oral cancer and risk factors, and performing routine oral examinations were very important for the early diagnosis of SCC.
