Acil tıp kliniğine başvuran ve ortopedi ve travmatoloji konsültasyonu yapılan erişkin adli olguların geriye dönük analizi

Abstract:

Amaç: Çalışmada yetişkin yaş grubunda adli olaylara bağlı meydana gelen yaralanmaların ortopedi ve travmatoloji bilimi açısından özelliklerinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Tepecik Eğitim ve Araştırma Hastanesi Acil Servisi’ne 01.01.2016–31.12.2016 tarihleri arasında başvurmuş ve ortopedi ve travmatoloji kliniğine konsülte edilmiş 182 hasta değerlendirilmiştir. Hastaların demografik özellikleri, acil servise başvuru şekli, travma şekli, meydana gelen yaralanmanın özellikleri, acil serviste istenilen konsültasyonları, radyolojik görüntülemeleri, yatış durumları değerlendirilmiştir. Bulgular: Hastaların %15’i (n= 27) kadın olup yaş ortalaması 46 (±18)‘ydı ve %85’i erkek olup yaş ortalaması 38 (±14)’di. Ortopedi kliniğine konsülte edilen adli olguların çoğunluğunda yaralanma şeklinin ateşli silah yaralanmaları olduğu, bunu sırasıyla şiddete bağlı künt travma, yüksekten düşme, kesici delici alet yaralanmalarının takip ettiği görüldü. Kadınlarda en sık yaralanma şekli düşme, erkeklerde ateşli silah yaralanmasıydı. Hastaların %45’inde (n= 81) kemik kırığı meydana gelmişti. Yaralanma bölgesinin en sık üst ekstremiteler (n= 92), bunu alt ekstremite (n= 75), baş boyun (n= 21), batın (n= 20) bölgelerinin takip ettiği görüldü. Sonuç: Ülkemizde, ortopedi ve travmatoloji kliniklerini ilgilendiren ve adli olay niteliği taşıyan yaralanmalara, yetişkin erkeklerin kadınlara kıyasla daha fazla oranda maruz kaldığı görülmektedir. Ortopedi ve travmatoloji konsültasyonu ihtiyacı olan 19 ve üzeri yaş grubundaki adli olguların büyük çoğunluğunda yaralanmanın ateşli silahlarla meydana geldiği; 40 yaşına kadar, ateşli silah yaralanmalarının en fazla olduğu, daha sonraki yaş gruplarında şiddete bağlı künt travma ve düşme sonucu yaralanmaların ön plana çıktığı görülmektedir. Çalışmamızda büyük damar yaralanması meydana gelen 8 hastanın 7’sinde yaşamı tehdit edici bir durumun meydana gelmediği yönünde karar verilmiş olması, hastaların %20’sinde yumuşak doku yaralanması boyutlarının tanımlanmamış olması, düzenlenen adli raporlarda eksiklikler olduğunu göstermektedir.

Returning analysis of adult judicial facts applicant to the emergency medical clinic and consulting with ortopedians and traumatologists

Abstract:

Purpose: The study aims to investigate the characteristics of injuries arising from judicial events in the adult age group from the point of view of the ortopedic and traumatological science. Tools and Methods: Reactive Education and Research Hospital Emergency Service has been applied between 01.01.2016 and 31.12.2016 and 182 patients consulted in the ortopedic and traumatological clinic have been evaluated. The demographic characteristics of the patients, the form of application for emergency service, the form of trauma, the characteristics of the injury occurring, the consultations requested in the emergency service, the radiological images, the condition of residence have been evaluated. Results: 15% of patients (n=27) were women with an average age of 46 (±18) and 85% were men with an average age of 38 (±14). In most of the judicial cases consulted at the ortopedic clinic, the form of injury was seen as firearm injury, followed by violent count trauma, falling from height, cutting-edge instrument injury, respectively. Women were injured, men were injured with firearms. 45% of patients (n=81) had bone breaks. The upper limbs of the wound area (n=92), followed by the lower limbs (n=75), the head neck (n=21), and the western (n=20). The result: In our country, injuries involving ortopedian and traumatological clinics and having the nature of a judicial event are seen to be more exposed to adult men than women. The majority of the legal incidents in the age group of 19 and above that need ortopedian and traumatological consultation, the injury occurs with firearms; up to the age of 40, the injury with firearms is overwhelmed, the injury resulting from violent knee trauma and falling in the subsequent age groups appears to come to the forefront. Our study shows that 7 of 8 patients with major vascular injuries have been determined that no life-threatening condition has occurred, 20% of patients have not identified the size of soft tissue injuries, and there are deficiencies in the judicial reports.

Retrospective Analysis Of Adult Forensic Cases Admitted To The Emergency Medicine Clinic and Consulted With Orthopedics and Traumatology

Objective: The aim of this study was to investigate the characteristics of the injuries caused by forensic events in the adult age group in terms of orthopedics and traumatology science. Materials and Methods: One hundred eighty two patients admitted to the Emergency Department of Tepecik Training and Research Hospital between 01.01.2016-31.12.2016 and consulted to orthopedy clinic were evaluated. Demographic characteristics of patients, admission to the emergency department, trauma type, characteristics of the injuries, additional consultations, radiological imaging methods, hospitalizations were evaluated. Results: 15% (n=27) of the patients were female, the mean age was 46±(18). The majority of forensic cases who were consulted to the orthopedy clinic were gunshot injuries followed by blunt trauma, fall from height, stab wounds respectively. The most common type of injury was falling in women, gunshot in men. Bone fractures occurred in 45% (n= 81) of the patients, most common site of injury was upper extremities (n= 92), followed by lower extremity (n= 75). Conclusion: In our country, it is seen that adult men are more exposed to forensic injuries than women. In the majority of forensic cases who needed orthopedy consultation up to the age of 40, the number of firearm injuries was the highest, subsequent injuries caused by blunt trauma due to violence and falls were prominent. In our study, it was decided that 7 of 8 patients with major vascular injuries did not develop a life-threatening condition, 20% of the patients did not have the dimensions of soft tissue injury. This situation shows that there are deficiencies in the judicial reports.
