Romatoid Artritli Hastalarda Serum İnterleukin 1α Düzeyleri ile İnsülin Direnci Arasındaki İlişki

Dolaşımdaki normal düzeylerde insüline hedef hücrenin cevabında bozulma olarak tanımlanan insülin direnci, sık rastlanan patolojik bir durumdur. Sitokinlerin insülin duyarlılığı üzerindeki potansiyel etkileri yoğun araştırmalara konu olmuştur. Bu çalışmanın amacı, romatoid artritli hastalarda insülin direnci gibi glukoz metabolizma anormallikleri ile serum interleukin 1α düzeyleri arasındaki bağlantıyı araştırmaktır. Çalışmaya değişik derecelerde hastalık aktivitesine sahip romatoid artritli 36 hasta (22 kadın ve 14 erkek, ortalama yaş: 51.5 ±17.1 yıl (21-80 yaş arası), ortalama VKİ: 27.1±5.0 kg/m²) ve yaş, cinsiyet, VKİ olarak benzer özelliklere sahip 20 sağlıklı kontrol dahil edilmiştir. Açlık serum IL-1α ve insülin düzeyleri ölçüldü. İnsülin duyarlılığı HOMA yöntemiyle hesaplandı. Romatoid artritli hastalarda kontrollere oranla açlık serum insülin düzeyleri, HOMA skorları ve serum IL-1α düzeyleri anlamlı derecede yüksekti (14.7±6.7, 8.7±1.9 µIU/ml, p<0.001 ve 3.3±1.5, 1.9±0.5, p<0.001 ve 18.2±13.3, 13.0±2.4 pg/ml, p<0.01 sırasıyla). Tüm olgularda açlık insulin, HOMA-IR, IL-1α düzeyleri arasında bağlantı mevcuttu. Çalışmamızda, romatoid artritli hastalarda IL-1α regülasyonunun bozulduğunu, IL-1α ile insülin duyarlılığı arasında anlamlı derecede bağlantı bulunduğunu gözlemledik.

Relationship Between Serum Interleukine Level 1α and Insulin Resistance in Patients with Romatoid Arthritis

Insulin resistance, defined as disruption in the response of the target cell to normal levels of circulation, is a common pathological condition. The potential effects of cytokines on insulin sensitivity have been subject to intense research. The aim of this study is to explore the connection between glucose metabolism abnormalities such as insulin resistance in patients with rheumatoid arthritis and serum interleukin 1α levels. The study included 36 patients with rheumatoid arthritis with disease activity in different degrees (22 women and 14 men, average age: 51.5 ±17.1 years (21-80 years), average VKI: 27.1 ±5.0 kg/m2) and 20 healthy controls with similar characteristics as age, gender, VKI. The hunger serum levels of IL-1α and insulin were measured. The sensitivity of insulin was calculated by the HOMA method. In patients with rheumatoid arthritis, the levels of hunger serum insulin, HOMA scores and serum IL-1α levels were significantly increased (14.7±6.7, 8.7±1.9 μIU/ml, p<0.001 and 3.3±1.5, 1.9±0.5, p<0.001 and 18.2±13.3, 13.0±2.4 pg/ml, p<0.01 respectively). In all cases there was a connection between the levels of hunger insulin, HOMA-IR, IL-1α. In our study, we observed that in patients with rheumatoid arthritis, the regulation of IL-1α was deteriorated, there was a significant link between IL-1α and insulin sensitivity.
