Amaç: Bu çalışmanın amacı, Çanakkale Devlet Hastanesi kapsamında yürütülen Evde Sağlık Hizmetlerinden (ESH), Ocak 2015-Aralık 2017 tarihleri arasında faydalanmış ve/veya faydalanmakta olan bireylerin sosyo-demografik özelliklerinin ve bu hizmet gereksinimine neden olan psikolojik ve hastalık durumların belirlenmesidir. Yöntem: Tanımlayıcı tipte bu epidemiyolojik araştırma kapsamında Ocak 2015-Aralık 2017 tarihleri arası Çanakkale Devlet Hastanesi Evde Sağlık Birimi tarafından sağlık hizmeti sunulan 901 kişinin takip dosyaları incelendi. Dosyalarda bulunan bilgiler üzerinden veri toplama formu hazırlandı ve kişilerin sosyodemografik özellikleri, takip süreleri, yatağa bağımlılık, kişisel bakım, güvenlik, kişisel hijyen, beslenme, yardımcı araç kullanımı ve psikolojik durumları, kronik hastalık, geçirilmiş ameliyat, bası ülseri, nöroloji, dahiliye konsültasyonu varlığı, şu an takipte olmayan bireylerin takipten çıkış nedenleri kayıt altına alındı. Bulgular: İncelenen kişilerin %62.7’si kadın, yaş ortalaması 77.72±12.78 yıldı. Kişilerin %40.7’si tam bağımlıydı, %26.7’sinin beslenmesi uygun değildi, psikolojik durum değerlendirilmesinde %44.9’u üzüntülüydü, %13.1’inde bası ülseri vardı, %7.7’si yardımcı araç kullanıyordu, %99.3’ünde en az bir kronik hastalık vardı, %22.8’inin ameliyat öyküsü vardı. Sonuç: Kişilerin fiziksel ve psikososyal sağlıklı durumlarının devamı için birinci basamak sağlık hizmeti sunan birimlerde ve ESH birimlerinde hizmet modeli içinde diyetisyene, psikoloğa ve psikiyatri uzmanına, sosyal hizmet çalışanlarına ihtiyaç vardır. ESH alan kişilerin tedavi başarıları ve bu hizmetin verimliliği için ESH birinci basamak sağlık hizmetleri ile entegre ve eşgüdümlü olmalıdır. ESH ihtiyacı olan kişilere yönelik hizmet öncelikle birinci basamak sağlık hizmetleri tarafından belirli aralıklarla takip edilecekleri 65 yaş üzeri izlem modeli oluşturmaktır.
Purpose: The purpose of this study is to identify the socio-demographic characteristics of individuals who have been benefited and/or being benefited from the Home Health Services (ESH) within the framework of the Çanakkale State Hospital and the psychological and disease conditions that lead to the need for this service between January and December 2017. Method: In the framework of this epidemiological research in the identificative type, the tracking files of 901 people provided health services by the Home Health Unit of the Çanakkale State Hospital between January 2015-December 2017 were examined. The data collection form was prepared through the information contained in the files and the social demographic characteristics of the persons, tracking times, bed addiction, personal care, safety, personal hygiene, nutrition, assistant vehicle use and psychological conditions, chronic illness, carried out surgery, press release, neurology, the presence of internal consultation, the reasons for the exit from tracking of the persons not currently tracking were recorded. Results: 62.7% of the respondents were women, the average age was 77.72±12.78 years. 40.7% of the people were completely addicted, 26.7% were inadequate in nutrition, 44.9% were sad in the psychological status assessment, 13.1% had press releases, 7.7% were using assistant vehicles, 99.3% had less chronic illness, 22.8% had a surgical history. The result: for the continuation of the physical and psychosocial health of individuals, the first-level health care facilities and the ESH facilities require a dietologist, a psychologist and a psychiatrist, a social service staff within the service model. For the therapeutic success of those who receive ESH and the efficiency of this service, ESH must be integrated and coordinated with first-level health care services. The service for those in need of ESH is to create a monitoring model for people over the age of 65 who will be followed by first-level health services at certain intervals.

A questionnaire was prepared on the information in the files. Sociodemographic characteristics, follow-up periods, bed dependence, personal care, safety, personal hygiene, nutrition, use of auxiliary tools and psychological conditions, chronic diseases, previous surgery, pressure ulcer, neurology, internal medicine consultation, the reasons for the follow-up was recorded. Results: It was found that 62.7% were female, mean age was 77.72 ± 12.78, 40.7% were fully dependent, 26.7% of them were mis-nutrition, 44.9% of them were sad, %13.1 had press ulcer, 7.7% were using auxiliary tools, 99.3% had at least one chronic disease, 22.8% had operation. Conclusion: In order to maintain the physical and psychosocial health status of individuals, there is a need for dieticians, psychologists and psychiatrists and social workers in the service model in primary health care units and in HHS units. HHS should be integrated and coordinated with primary health care services for the success of treatment and the efficiency of this service. The service for people in need of HHS is primarily to create a follow-up model over 65 years of age, followed by primary health care.
