Amaç: Çalışmamızda Herediter Multipl Egzostozis (HME) hasta grubunda başta ağrı olmak üzere hastalığa ait komplikasyonların çocuk ve adölesan dönemdeki etkilerini değerlendirmeyi amaçladık. Hastalar ve Yöntem: 10-19 yaş arası 72 (37Erkek/ 32Kadın) hasta değerlendirmeye dahil edildi. Ortalama yaş 13.8±2.6 idi. Hastaların demografik verileri, lezyonların ilk ortaya çıktığı yaş, lezyon sayıları, deformite varlığı ve VAS skoru ile ağrı durumları değerlendirildi. Çalışmaya katılan hastalara, ağrının yaşam kalitesi üzerine etkilerini değerlendirmeye yönelik bir anket yöneltildi. Depresyon değerlendirmesi 17 yaş ve altı hastalarda Çocuklar için Depresyon Ölçeği (ÇDÖ) ve 18 yaş ve üstü hastalar için Hamilton Depresyon Ölçeği (HAM-D) ile değerlendirildi. Grup değişkenlerinin frekans ve yüzdelik değerleri, iki grup ortalaması arasındaki farkları belirlemede “t-testi” ikiden fazla ortalamanın karşılaştırılmasında tek yönlü varyans analizi kullanıldı Bulgular: Hastaların %68.1’i (n=49) HME nedeniyle bir veya daha fazla cerrahi girişim geçirmiş olgulardan oluşmaktaydı. Ağrı, hastaların %72.2’sinde (n=52) en belirgin şikayet idi. Ağrıyı başlatan en sık sebepler hastaların %52.8’inde koşma iken, %50’sinde kontakt sporlar (futbol, basketbol vb.) idi. Hastaların %31.9’u ağrının günlük yaşam aktivitelerini kısıtlayabildiğini bildirirken ortalama tüm gruplarda ortalama VAS skoru 7.4±2.4 idi. VAS skoru azaldıkça ÇDÖ puanlarında anlamlı bir artış tespit edildi. (p=0.005) Yüksek ÇDÖ ve HAM-D puanlarının geçirilen ameliyat sayısı (p=0.015), bir aydaki ağrılı gün sayısı (p<0.001), düşük VAS skoru (p=0.005) ve ekstremitelerde yer alan lezyon sayıları (p=0.003) ile anlamlı olarak ilişkili olduğu belirlendi. Sonuç: Ağrı pediatrik HME hastalarında sosyokültürel aktiviteleri ve etkileşimi kısıtlayabilen en önemli sorunlardan biridir. Geçirilen ameliyat sayısı, bir ayda yaşanan ağrılı gün sayısı, düşük VAS skorları ve artmış lezyon sayıları HME hastalarında yaşam kalitesini negatif etkileyen ve pediatrik yaş grubunda depresyona yol açabilen faktörlerdir. HME hastalığın süresi pediatrik hastalarda depresyon ile ilişkilidir.
Purpose: In our study, we aimed at evaluating the effects of complications of the disease, including pain in the Hereditary Multiple Exostosis (HME) group of patients in the childhood and adolescence period. Patients and Method: 72 (37 Men/32 Women) between 10 and 19 years of age were included in the patient assessment. The average age was 13.8 ± 2.6. The demographic data of patients, the age in which the lesions first occurred, the number of lesions, the presence of deformity and the pain conditions with the VAS score were evaluated. Patients involved in the study were asked to evaluate the effects of pain on the quality of life. Depression assessment was assessed by the Depression Scale for Children in the ages of 17 and 6 and Hamilton Depression Scale (HAM-D) for patients aged 18 and older. The frequency and percentage values of the group variables were "t-test" in determining the differences between the two group average; in comparison of more than two average; one-way variance analysis was used findings: 68.1% of patients (n=49) were made up of one or more surgical interventions due to HME. The pain was the most noticeable complaint in 72.2% of patients (n=52). Patients and Methods: 72 (37male/32female) patients aging between 10-19 years were included in study. The mean age was 13.8±2.6 years. The demographic data of the patients, age at diagnosis, number of lesions, deformity and pain with VAS score were evaluated. The quality of life was evaluated with the use of a question-naire. Child Depression Scale (CDS) for the patients who were 17 years or younger and Hamilton Depression Scale (HAM-D) for the patients who were 18 or older, were used to evaluate the mood. The frequency and percentage of the group variables were noted, "t-test" was used to determine the differences between the two groups; one-way analysis of variance was used to compare more than two averages. Results: In our study 68.1% (n=49) of the patients had one or more surgical interven-tions due to HME. The most common complaint was pain in 72.2% (n=52) of patients. The most common cause that initiated pain was running 52.8% and contact sports (football, basketball, etc.) was reported in 50% of patients. 31.9% of the patients reported that the pain limited their daily activities, the mean VAS score was 7.4 ±2.4. Decreased VAS scores were significantly related with increased CDS scores. (p=0.005) Higher CDS and HAM-D scores were significantly related with the number of operations (p=0.015), the number of days with pain per month (p <0.001), lower VAS scores (p=0.005) and the number of lesions in the extremities (p=0.003).. Conclusion: Pain is a crucial problem that can limit sociocultural activities and inter-action in pediatric HME patients. The number of previous operations,number of days with pain per month and increased number of lesions are factors that negatively affect the quality of life and cause depression in HME patients.
