Bu araştırma postpartum depresyonun maternal bağlanmaya etkisinin incelenmesi amacıyla longitudinal analitik bir çalışma olarak planlanmıştır. Araştırma kapsamına 01 Ocak-28 Şubat 2011 tarihleri arasında T.C. Manisa Merkez Efendi Devlet Hastanesi Doğum ve Çocuk Kliniği’nde doğum yapan, araştırma kriterlere uyan ve araştırmaya katılmayı kabul eden 100 kadın alınmıştır. Veriler üç aşamada toplanmıştır. Kadınlara birinci aşamada doğum sonrası Kadınların Sosyo-demografik ve Doğurganlık Özellikleri Soru Formu uygulanmıştır. Doğum sonu birinci ve üçüncü ay kadınlara ev ziyareti yapılmış, Doğum Sonrası Depresyon Tarama Ölçeği (DDTÖ) Formu ve Maternal Bağlanma Ölçeği (MBÖ) Formu uygulanmıştır. Araştırmada kadınlara yapılan ev ziyaretleri 01 Haziran 2011 tarihinde tamamlanmıştır. Verilerin analizlerinde SPSS 17.0 istatistik paket programı kullanılmıştır. Kruskal Wallis, Mann Whitney U, bağımsız ve bağımlı gruplarda t-test ve pearson korelasyon testleri istatistiksel analizlerde kullanılmıştır. Araştırmaya katılan kadınların MBÖ’den aldıkları toplam puan ortalaması doğum sonrası birinci ayda 95.1±13.3, üçüncü ayda 96.6±13.0 bulunmuştur. Postpartum birinci ve üçüncü ay MBÖ toplam puan ortalamaları arasında istatistiksel olarak anlamlı bir fark vardır (p<0.05). Bu çalışmada doğum sonu depresyon birinci ayda %19.0, üçüncü ayda %10.0 olarak bulunmuştur. Anne yaşının küçük olması DSD riskini arttırmaktadır. Kadınların DDTÖ toplam puan ortalaması postpartum birinci ay 62.1±25.3, postpartum üçüncü ay 51.9±20.7 olup, iki grup arasında istatistiksel olarak anlamlı bir fark saptanmıştır (p<0.05). Postpartum birinci ve üçüncü ayda MBÖ toplam puanı ile DDTÖ toplam puanı arasında iyi derecede negatif yönde bir korelasyon bulunmuştur. Doğum sonu depresyon puanı yüksek olan annelerde maternal bağlanma puanı düşük belirlenmiştir. DSD, anne-bebek etkileşimini olumsuz yönde etkilemektedir. Sağlık çalışanları annelerin postpartum dönemde psikolojik durumunu ve maternal bağlanmaya etkisini değerlendirmelidir.
This study is planned as a longitudinal analytical study aimed at studying the effects of postpartum depression on maternal bonding. Research scope from 01 January to 28 February 2011 to T.C. 100 women who have been given birth in the Manisa Central Lord State Hospital in the Birth and Child Clinic, who comply with the research criteria and agree to participate in the research. The data is collected in three stages. In the first phase, women were applied the Socio-Demographic and Fertility Characteristics Questionnaire Form for Postpartum Women. The first and third months of birth were home visits to women, the postpartum depression screening scale (DDT) form and the maternal connectivity scale (MBÖ) form were applied. The study conducted home visits to women was completed on June 1, 2011. The SPSS 17.0 statistical package program is used for data analysis. Kruskal Wallis, Mann Whitney U, t-test and pearson correlation tests in independent and dependent groups have been used in statistical analysis. The total score of women who participated in the study was 95.1±13.3 in the first month after childbirth, 96.6±13.0 in the third month. There is a statistically significant difference between the total score average of the first and third months of postpartum (p<0.05). In this study, birth end depression was 19.0% in the first month and 10.0% in the third month. The age of the mother increases the risk of DSD. The total score for women is 62.1±25.3 in postpartum first month, and 51.9±20 in postpartum third month. There is a statistically significant difference between the two groups (p<0.05). In the first and third months of postpartum, there was a correlation in a good negative direction between the total score of MBÖ and the total score of DDÖ. In mothers with a high depression score, the maternal bonding score is low. DSD affects mother-baby interaction in a negative way. Health workers should evaluate the psychological state of the mothers in the postpartum period and the impact on maternal bonding.
