Amaç: Bu çalışmada, İzmir ilinde gebelerde sitomegalovirüs (CMV) seroprevalansını belirlemek ve CMV IgG antikor düzeyleri ile birlikte CMV avidite test sonuçlarını yıllara göre değerlendirmek amaçlandı. Yöntem: Ocak 2016-Aralık 2019 tarihleri arasında, üniversite hastanesi mikrobiyoloji laboratuvarında 20. gebelik haftası ve altındaki gebelerin (≥18 yaş) serum örneklerinde kemiluminesan mikropartikül immunoassay yöntemi ile çalışılan CMV IgM, CMV IgG ve CMV IgG avidite test sonuçları retrospektif olarak analiz edildi. Bulgular: Çalışmaya dâhil edilen 3.062 gebenin yaş ortalaması 28.57±5.96 yıl (aralık: 18-48 yaş) idi. CMV IgG ve IgM seropozitiflik oranı sırasıyla %94.22 (n=2885/3062) ve %1.37 (n=42) olarak bulundu. CMV IgM pozitif olan üç hastada CMV IgG avidite indeksi düşük belirlendi (%7.14, n=3). CMV seropozitif gebelerin IgG antikor düzeyi ve yaşları arasında zayıf bir korelasyon ilişkisi saptandı (p=0.041, ρ=0.038, r2=0.001). CMV IgG seropozitiflik oranı yıllara göre değişmekle birlikte, 2016 yılında (%83.22) en düşük düzeyde, CMV IgM pozitiflik oranı ise 2019 yılında (%2.13) en yüksek düzeyde saptandı. CMV seropozitifliği, 38-48 yaş arası gebelerde en yüksek düzeyde saptandı ve yaşa göre bu grupta CMV IgG düzeylerinin anlamlı olarak daha düşük olduğu görüldü (p=0.004). Sonuç: Çalışmada, İzmir ilindeki gebelerde CMV IgG seroprevalansı (%94.22) yüksek seyretmektedir, ancak seronegatif ve CMV IgM pozitif gebelerin de varlığı konjential CMV enfeksiyonu açısından riskli gebeliklerin devam ettiğini göstermektedir. Etkin bir ulusal sürveyans ağının oluşturulmasıyla seroprevalans çalışmalarının kapsamının genişletilerek bölgelere ve yıllara göre sürekli olarak güncellenen verilerle gebe takibinde ek tedbirlerin uygulanması sağlanabilir.
Purpose: In this study, it was intended to determine the seroprevalence of cytomegalovirus (CMV) in pregnancies in the province of Izmir and to evaluate the results of the CMV avidite test according to years along with the levels of CMV IgG antibodies. Method: Between January 2016-December 2019, the results of the CMV IgM, CMV IgG and CMV IgG avidite tests, which were studied in serum samples of the 20th week of pregnancy and under pregnancy (≥18 years of age) in the university hospital microbiology laboratory, were analyzed retrospectively. Results: The average age of 3,062 students included in the study was 28.57±5.96 years (December: 18-48 years). The CMV IgG and IgM seropositivity rates were 94.22% (n=2885/3062) and 1.37% (n=42) respectively. In three patients with CMV IgM positive, the CMV IgG avidite index was low (%7.14, n=3). CMV seropositive pregnancies had a weak correlation between the IgG antibodies level and age (p=0.041, ρ=0.038, r2=0.001). Although the CMV IgG seropositivity rate varied over the years, it was at the lowest level in 2016 (83.22) and the CMV IgM positive rate at the highest level in 2019 (2.13) CMV seropositivity was found at the highest levels in pregnancies between 38 and 48 years of age, and it was found that CMV IgG levels in this age group were significantly lower (p=0.004). Result: In the study, CMV IgG seroprevalence in pregnancies in the Izmir province (94%). 22) looks high, but the presence of seronegative and CMV IgM positive pregnancies also indicates that congenital CMV infection risk pregnancies continue. With the creation of an effective national surveillance network, the scope of seroprevalence studies can be extended and the implementation of additional measures in pregnancy tracking with data that are constantly updated by regions and years.
