Konjonktival Lezyonların Histopatolojik Analizi: 10 Yıllık Deneyim

Abstract:

Amaç: Konjonktiva; çeşitli neoplastik yada nonneoplastik lezyonların gelişebileceği bir bölgedir. Bu çalışmada amaç konjonktival lezyonların histopatolojik olarak analizini yapmak ve prevalansını tespit etmektir. Gereçler ve yöntem: 2009-2019 yılları arasında patoloji laboratuarına gelen 401 olguya ait konjonktiva biyopsileri retrospektif olarak incelendi. Olgular histopatolojik olarak nonneoplastik ve neoplastik lezyonlar olarak 2 gruba ayrıldı. Neoplastik lezyonlar benign, premalign ve malign lezyonlar olarak alt gruplara bölündü. Lezyonların görülme oranları ile gruplara göre yaş ve cinsiyet dağılımı analiz edildi. Bulgular: 401 olguya ait serimizde yaş ortalaması 49.89±21.75 olup olguların 209 (%52.1) u erkek, 192 (%47.9) i kadın idi. 296 (%73.8) olguda nonneoplastik,105 (%26.2) olguda neoplastik lezyon tespit edildi.  Nonneoplastik lezyonlarda yaş ortalaması 50.79±19.34, neoplastik lezyonlarda ise 47.34±27.39 idi. Neoplastik lezyonlardan 64’ü (% 60.2) benign,  17’si (%16.2) premalign, 24’ü (%22.6) malign idi. Pterygium (n:220; %54.78) en sık görülen nonneoplastik lezyon, nevüsler ve diğer pigmente lezyonlar (n:38; %9.5) en sık görülen benign lezyon, skuamöz hücreli karsinom (n:14; %3.5)  ise en sık görülen malign lezyon olarak tespit edildi. Neoplastik lezyonların görülme sıklığı 45 yaş üzerinde anlamlı olarak artmakta idi (p<0.05). Sonuç: Mevcut çalışmada; laboratuvarımızda10 yıl boyunca tanı almış olan konjonktival lezyonlar gözden geçirilerek histopatolojik analizi yapıldı. Konjonktival lezyonların tanınması, nonneoplastik lezyonlar ve benign lezyonlar ile premalign ve malign lezyonların oranlarının farkında olunması, tedavinin planlanması ve görme fonksiyonlarının korunması bakımından son derece önemlidir.

Purpose: Conjunctiva; It is a region where various neoplastic or nonneoplastic lesions can develop. The aim of this study is to analyze the conjunctival lesions histopathologically and to determine their prevalence. Materials and methods: Conjunctival biopsies of 401 cases who came to the pathology laboratory between 2009 and 2019 were retrospectively analyzed. The cases were histopathologically divided into 2 groups as nonneoplastic and neoplastic lesions. Neoplastic lesions were subdivided into benign, premalignant and malignant lesions. The incidence of lesions and the distribution of age and gender according to the groups were analyzed. Results: In our series of 401 cases, the mean age was 49.89±21.75 years, with 209 (52.1%) males and 192 (47.9%) females. Nonneoplastic in 296 (73.8%) cases, 105 (26%. 2) Neoplastic injury was detected in the case.  The average age in nonneoplastic lesions was 50.79±19.34, and in neoplastic lesions 47.34±27.39. Of the neoplastic lesions, 64 (60.2%) were benign, 17 (16.2%) premalign, 24 (22.6%) malign. Pterygium (n:220; 54.78) was the most common nonneoplastic injury, neurotic injury and other pigment injury (n:38; 9.5%) and the most common benign injury, and squamous cell carcinoma (n:14; 3.5%) was the most common malign injury. The incidence of neoplastic injury increased significantly over the age of 45 (p<0.05). Result: In the current study, the conjunctival lesions that have been diagnosed in our laboratory for 10 years were reviewed and a histopathological analysis was carried out. The recognition of conjunctive lesions, the recognition of nonneoplastic lesions and benign lesions and the rates of premalign and malign lesions, the planning of treatment and the preservation of visual functions, are extremely important.
