Tromboembolizm sık olarak görülebilen fakat güç teşhis edilen, morbiditesi ve mortalitesi yüksek olan bir hastalıktır. Tromboembolizm için tanımlanan birçok risk faktörü bulunmaktadır. Ancak herediter faktörler, özellikle tekrarlayan venöz trombozlu olgular için önemli bir risk faktörüdür. Protein S (PS), K vitamini ilişkili bir antikoagülandır. PS, aktif protein C’nin (APC), kendi substratları olan aktif faktör V ve aktif faktör VIII üzerine olan etkilerini kolaylaştıran bir kofaktördür. PS eksikliği, klinik olarak tromboz ile ilişkilidir. Bu yazıda, PS eksikliği olan 38 yaşında bir kadın hastada genel anestezi uygulaması ve sonuçları sunuldu. Ameliyat öncesi 1500 U.gün-1 heparin ve bandaj uygulaması yapıldı. Ameliyat sonrası birinci gün mobilizasyon teşvik edildi. Bu olguda, heparin premedikasyonu, alt ekstremitelerin aktif mobilizasyonu ve elastik bandaj uygulamasının, tromboz ve embolizmi önlemede etkili olduğu görüldü. Anestezistler PS eksikliği olan hastalarda, anestezi süresince ve sonrasında oluşabilecek tromboz riskine karşı dikkatli olmalıdırlar.
Thromboembolism is a disease that can be seen frequently but is diagnosed with a high morbidity and mortality. There are many risk factors that are defined for tromboembolism. However, hereditary factors are an important risk factor, especially for repeated venous thrombosis events. Protein S (PS) is an anticoagulant associated with vitamin K. PS is a cofactor that facilitates the effects of active protein C (APC), its own substrates on active factor V and active factor VIII. PS deficiency is clinically associated with thrombosis. In this article, a 38-year-old woman with PS deficiency presented general anesthesia application and results. 1500 U-1 days before the surgery were applied heparin and bandage. On the first day of the operation, the mobilization was encouraged. In this phenomenon, premedication of heparin, active mobilization of the lower limbs and the application of elastic binding, has been shown to be effective in preventing thrombosis and embolism. Anesthesians should be cautious with the risk of thrombosis that may occur during and after anesthesia in patients with PS deficiency.
