The Relationship Between Frontal Osteoma and Frontal Sinus Size and Its Effect on the Choice of Surgical Method

Objectives: The size and function of the sinuses are influenced by many factors. For this purpose, we planned to investigate the relationship between frontal osteoma dimensions, localization, and presence of osteoma and frontal sinus size and the rate of surgical intervention. Materials and Methods: After approval of the ethics committee, the archive images of 146 patients with frontal osteoma detected by 27616 paranasal sinus CTs taken between 2014-2019 were retrospectively reviewed. The distance between the most medial and lateral points of the frontal sinus was measured by coronal and horizontal measurements;the distance between the most cephalic and caudal points and the horizontal line drawn to the upper boundary of the orbital distance was measured with reference to the most caudal distance. We investigated whether there was a significant relationship between osteoma size and frontal sinus size and going to surgery. Results: There was no significant difference between the right and left frontal sinus dimensions of the patient and control groups. 86.3% of the osteoma group was found to be stage 1. In the whole osteoma group only 8.2% had undergone surgical intervention. The measured osteoma width, height and height values of male patients were higher than female patients. There was no difference between the percentages of osteoma, osteoma size and right and left frontal sinus sizes according to osteoma side of the patients. While a positive correlation was observed between the sinus widths of the patients with osteoma and sinus widths without osteoma, sinus widths with osteoma and sinus length without osteoma (r=0.400, p<0.001) and heights. Conclusion: The presence of an osteoma does not negatively affect sinus dimensions on the involved side and the opposite side. As long as the osteoma is not enlarged, it may be asymptomatic for a long time. The choice of surgical approach depends on the location and size of the osteoma.
