Amaç: İnvazif kandidozda erken tanı çok önemlidir ve mortalite oranını düşürmek için kısa sürede ve güvenilir sonuç veren yöntemlere ihtiyaç duyulmaktadır. Bu çalışmada, Candida türlerinin daha hızlı tanımlanabilmesi için lizis filtrasyon sonrası MALDI-TOF MS yönteminin uygunluğu araştırılmıştır. Yöntem: Bu çalışmada, Ege Üniversitesi, Tıp Fakültesi, Tıbbi Mikrobiyoloji Laboratuvarına gönderilen kan kültürlerinden izole edilen 100 Candida kökeni, Dalmau plak, MALDI-TOF MS ve lizis filtrasyon sonrası MALDI-TOF MS yöntemleriyle araştırıldı ve elde edilen sonuçlar karşılaştırıldı. Bulgular: Çalışmamızda konvansiyonel ve MALDI-TOF MS tanımlama yöntemlerine göre; 37 Candida albicans, 23 Candida parapsilosis, 17 Candida tropicalis, dokuz Candida glabrata, altı Candida kefyr, üç Candida guilliermondii, üç Candida dubliniensis, üç Candida krusei olarak identifiye edilmiştir. Lizis filtrasyon sonrası MALDI-TOF MS yönteminde ise identifikasyon sonuçları; 26 C. albicans, dokuz C. parapsilosis, 13 C. tropicalis, dokuz C. glabrata, iki C. kefyr, üç C. dubliniensis, iki C. krusei olarak belirlenmiştir. MALDI-TOF MS yöntemi ve Dalmau plak ile lizis filtrasyon sonrası MALDI-TOF MS yöntemi 64 kökende uyumlu bulunmuştur. Uyumsuzluğun yanlış tanımlamadan değil, spektrum yetersizliğinden kaynaklandığı gözlemlenmiştir. Sonuç: “Spektrum yetersizliği” olarak tanımlanmayan suşlar veri tabanında yer alan türlerden oluşmaktadır. Altmış dört Candida türünün standart yöntemlerle %100 uyumlu olacak şekilde tanımlama yaptığı ve bu yöntemin subkültür gerektiren MALDI-TOF MS yöntemine göre en az 48 saat, Dalmau plak yöntemine göre de en az 72 saat avantaj sağladığı görülmüştür.
The early diagnosis of invasive candidiasis is very important and methods are needed to reduce the mortality rate in a short and reliable time. In this study, the conformity of the MALDI-TOF MS method after lizis filtration has been studied in order to identify Candida species faster. Method: In this study, 100 Candida origin, Dalmau plate, MALDI-TOF MS, and lizis after filtration, which was isolated from the blood cultures sent to the University of Ege, the Faculty of Medicine, the Medical Microbiology Laboratory, was studied with the MALDI-TOF MS methods and the results obtained were compared. The findings: according to the conventional and MALDI-TOF MS identification methods in our study; 37 Candida albicans, 23 Candida parapsilosis, 17 Candida tropicalis, nine Candida glabrata, six Candida kefir, three Candida guilliermondii, three Candida dubliniensis, and three Candida crosses were identified. In the MALDI-TOF MS method after Lizis filtration, the identification results were determined as: 26 C. albicans, nine C. parapsilosis, 13 C. tropicalis, nine C. glabrata, two C. kefyr, three C. dubliniensis, two C. crosses. MALDI-TOF MS method and Dalmau plate with lizis filtration after MALDI-TOF MS method have been found compatible with 64 roots. It has been observed that the incompatibility is not due to misdefinition, but to spectrum deficiency. The result is that the spots not defined as "spektral deficiency" consist of the species in the database. Sixteen and four Candida species have been defined to be 100% compatible with the standard methods, and this method has been shown to provide a benefit of at least 48 hours according to the MALDI-TOF MS method requiring subculture, and at least 72 hours according to the Dalmau plate method.
