Amaç: Bizim amacımız, elektif transüretral prosedür uygulanan hastalarda intratekal tramadolün bupivakaine eklenen fentanil ve bupivakaine eklenen plasebo ile karşılaştırıldığındaki anestezik ve analjezik etkilerini gözlemlemektir. Gereç ve Yöntem: Elektif transüretral prosedür uygulanan ASA I-III 146 hastanın dahil edildiği prospektif, çift kör, randomize çalışmada intratekal lokal anestetik ile tramadol HCL’nin anestetik ve analjezik etkinliğini değerlendirdik. Lomber intratekal blok, cerrahi için %0,5 Heavy Bupivakain tramadol HCL, fentanil veya salin ile kombine edilerek uygulandı. Her bir gruba, 12.5mg bupivakain ile 50μg (1ml) fentanil (F), 12.5mg bupivakain ile 10mg (1ml) tramadol (T) or 12.5mg bupivakain ile 1ml koruyucu serbest salin (S) verildi. Bulgular: Salin grubunda Tramadol HCL ve fentanil gruplarından daha çok hasta ağrı deneyimledi (p<0.05). Fentanil Grubunda tramadol ve salin gruplarıyla karşılaştırıldığında S1-2 segmentinden gerileme ortalaması daha uzundu (p<0.05). Komplikasyon görülme sıklığı benzerdi. Sonuç: Fentanil eklenen lokal anestestetiklerin postoperatif analjezi süresinin tramadol eklenen lokal anestetiklerle karşılaştırıldığında daha uzun olduğu sonucuna vardık.
Purpose: Our goal is to observe the anesthetic and analgesic effects of intratekal tramadol compared to fentanyl added to bupivakain and plasebo added to bupivakain in patients with elective transuretral procedure. Instrument and Method: The prospective, double-blind, randomized study in which ASA I-III 146 patients were involved in the elective transuretral procedure assessed the anesthetic and analgesic effectiveness of tramadol HCL with intratekal local anesthetics. Lomber intratekal block was applied for surgery by combining 0.5 % Heavy Bupivakain with tramadol HCL, fentanyl or salin. Each group was given 50μg (1ml) fentanyl (F) with 12.5mg bupivakain, 10mg (1ml) tramadol (T) or 12.5mg bupivakain with 1ml protective free saline (S). Results: Patients experienced more pain in the Salin group than Tramadol HCL and fentanyl groups (p<0.05). In the Fentanil group, the reduction average was longer than the S1-2 segment compared to the tramadol and salin groups (p<0.05). The frequency of complications was similar. The result: we found that the postoperative analgesic time of local anesthetics added to Fentanil was longer compared to the local anesthetics added to tramadol.
