Amaç: Yatarak kemoterapi alan kanserli çocuklarda mukozit gelişiminin beslenme durumlarına etkisini incelemektir. Bireyler ve Yöntem: Bu araştırma Hacettepe Üniversitesi Onkoloji Hastanesi Pediatrik Onkoloji servisine kemoterapi almak için yatan ve mukozit gelişen, 3-15 yaş aralığındaki (ortalama 7.5±4.1 yıl) 5 kız ve 8 erkek hasta üzerinde yürütülmüştür. Hastaların beşinde lenfoma, sekizinde solid tümör tanısı bulunmaktadır. Her hastanın kemoterapi öncesi (başlangıç noktası) ve mukozit oluştuğu gün (son nokta) antropometrik ölçümleri ve 24 saatlik besin tüketim kayıtları alınmış, biyokimyasal ve hematolojik bulguları değerlendirilmiş ve iştahları sorgulanmıştır. Bulgular: Kemoterapinin öncesinde ve mukozit başlangıcında hastaların kan bulguları (total protein, albümin, kreatinin, hemoglobin, hematokrit) değerlendirildiğinde, kan değerlerinin anlamlı ölçüde azaldığı görülmüştür (p<0.05). Ağırlık kaybı ortalaması %4.7’dir (p<0.05). Kemoterapi öncesi ve mukozit görülen gün için kendisini iştahsız olarak nitelendirenlerin oranı sırasıyla %7.7 ve %100.0’dür. Kemoterapi öncesi ve mukozit başlangıç günü için hastaların enerji, makro (protein, karbonhidrat, yağ) ve mikro (A, C, B6, B12 vitaminleri, folik asit, kalsiyum, demir, çinko ve bakır) besin ögeleri ile posa açısından, yaşlarına göre gereksinimlerini karşılamaları yüzdelerinin, istatistiksel açıdan anlamlı olarak düşük olduğu görülmüştür (p<0.05). Sonuç: Kemoterapi alan hastalarda mukozit görülmesi, tedavinin yan etkilerinden biridir. Mukozit, iştahı ve beslenmeyi etkileyerek, beslenme yetersizliğine ve ağırlık kaybına neden olmaktadır. Hastanın mukozit açısından değerlendirilmesi, beslenmenin izlemi ve yönetimi, kanser tedavisinin başarısını arttırmak açısından önemlidir.
Purpose: Study the effect of mucous development on nutritional conditions in children with cancer taking sleep chemotherapy. Individuals and Methods: This study was conducted on 5 girls and 8 male patients in the age range of 3-15 years (average 7.5±4.1 years) who were undergoing and developing mucous to receive chemotherapy to the Pediatric Oncology Service of the University of Hacettepe Oncology Hospital. Five patients have lymphoma and eight have a solid tumor diagnosis. Anthropometric measurements and 24-hour food consumption records were taken before chemotherapy of each patient (the starting point) and the day (the last point) of the formation of the mucosite, the biochemical and hematological findings were evaluated and the appetite was questioned. Results: Before chemotherapy and at the beginning of the mucosite, when patients’ blood finds (total protein, albumin, creatine, hemoglobin, hematocrit) were evaluated, the blood values were found to decline significantly (p<0.05). The weight loss average is 4.7 % (p<0.05). The percentage of those who describe themselves as appetite for the day prior to chemotherapy and the mucosite is 7.7% and 100%, respectively. For the pre-chemotherapy and mucosite start-day, the percentage of patients with energy, macro (proteins, carbohydrates, fats) and micro (A, C, B6, B12 vitamins, folic acid, calcium, iron, zinc and baker) nutrients and poses, according to their age, has been shown to be statistically significantly low (p<0.05). The result: the appearance of mucositis in patients receiving chemotherapy is one of the side effects of treatment. It affects the mucosite, appetite and nutrition, causing nutritional insufficiency and weight loss. The patient’s evaluation from the mucous point of view, the monitoring and management of the diet is important in order to increase the success of cancer treatment.
