Bu çalışmada yüksekten düşme öyküsü ile hastaneye müracaat ettirilen, ağır klinik bulgular ile öykü arasında çelişki olması nedeni ile adli tıbbi konsültasyon istenen çocuk olgu aktarılacak ve özellikle çocuklarda her kazanın altında istismar ve ihmalin yatabileceği olasılığına dikkat çekilecektir. Üç yaşında kız çocuk, öğle saatlerinde 4 metre yükseklikten beton zemine düşme öyküsü ile ülkemizde mülteci olan ailesi tarafından hastaneye müracaat ettirilmiştir. Ebeveynlerin küçük yaşta evlendiği ve işsiz oldukları anlaşılmıştır. İkiz kardeşi olan olgunun; bilincinin kapalı olduğu, saçlı deri sol temporo-parietal ödem ve alın sol tarafta abrazyonlar bulunduğu, göz kapaklarında hematom olduğu saptanmıştır. Tetkiklerinde; sağ femur kırığı, sol parietal kemikte kırık, sol orbita süperior kemik kırığı, sol lateral ventrikül komşuluğunda parankimal hemoraji ve sol akciğerde kontüzyon olduğu saptanmıştır. Yoğun bakımda; entübe ve sedatize edilmiş olduğu, genel fiziksel görünümü ve gelişiminin normal ayrıca hijyen bakımından normal bulunduğu, genital bölge muayenesinin normal olduğu saptanmıştır. Olgu her ne kadar ihmal ve istismar risk faktörlerini taşısa da; vücudunda değişik yaş ve şekillerde yaralar saptanmaması, oluşan yaraların yüksekten düşme ile oluşabilecek nitelikte olması, normal kiloda ve bakımlı görünmesi ve ailenin ilgili olması bizi istismardan uzaklaştırmıştır. Ancak tüm çocukların ebeveynlerinin bakım ve gözetimi altında olduğu düşünüldüğünde ihmal olduğu yönünde karar verilmiş ve sosyal inceleme raporu istenmiştir. Çocukluk çağı fiziksel travmaları iyi araştırılması gereken, ihmal ve istismar kaynaklı olabilecek ve multidisipliner yaklaşım sergilenmesi gereken durumlardandır. Bu konuda çocuk sağlığı profesyonelleri; önleme, raporlama ve tedavi etme aşamalarından çocuk koruma ve izleme hizmetlerine kadar pek çok aşamada önemli roller üstlenebilirler.
In this study, the story of falling from high to the hospital, due to the fact that there is a contradiction between severe clinical findings and the story, the child requested medical consultation will be transmitted and the possibility of abuse and impairment under each accident will be highlighted, especially in children. The three-year-old girl was hospitalized by her family, a refugee in our country, with the story of falling from 4 meters high into the concrete ground in the afternoon. Parents are married and unemployed at a young age. It is found that the twist brother of the phenomenon; that the consciousness is closed, that the hair skin is temporo-parietal on the left, and that the abrasions are on the left side of the neck, and that the hematom in the eye cover is found. In their examinations, it was found that there was a right femur breach, a left parietal bone breach, a left orbit superior bone breach, parankimal hemorrhage in the left lateral ventricle neighborhood, and contusion in the left lungs. In intensive care, it was found that it was entubated and sedated, its overall physical appearance and development was normal as well as hygienically normal, the genital area examination was normal. Although the fact carries the negligence and the risk factors of abuse; the lack of injuries in different ages and forms in the body, the quality of the wounds that can be formed by falling from the height, the normal weight and careful appearance and the family’s association have removed us from abuse. However, when all children are considered to be under the care and supervision of their parents, it was decided that they were neglected and a report of social examination was requested. Childhood physical trauma is one of the situations that should be well studied, which may be a source of negligence and abuse, and must be shown a multidisciplinary approach. In this regard, child health professionals can play important roles in many stages, from the prevention, reporting and treatment stages to child protection and monitoring services.
