Amaç: Sepsis olgularında yanlış antimikrobiyal tedavi alan veya tedavi almayan olgularda ölüm oranları kaybedilen her saat başına artış göstermektedir. Bu çalışmada, etkenin konvansiyonel kültür yöntemine oranla daha hızlı tanımlanabilmesi amacıyla konvansiyonel kan kültürü/MALDI TOF yöntemi ile lizis filtrasyon sonrası MALDI TOF MS (LFM) ve mikroarray yönteminin karşılaştırılması amaçlanmıştır. Yöntem: Kasım 2015-Şubat 2016 tarihleri arasında Ege Üniversitesi, Tıp Fakültesi Hastanesi, Erişkin Yoğun Bakım ünitelerinde izlenen hastalardan alınan kan kültürü örneklerindeki 39 Gram negatif etkene mikroarray ve LFM işlemi uygulanıp, ardından MALDI TOF sistemi ile çalışıldı. Bakteri kolonisinden yapılan MALDI TOF tür düzeyinde tanımlama ile lizis filtrasyon sonrası uygulanan MALDI TOF ve mikroarray test yöntemlerinin sonuçları karşılaştırıldı. Bulgular: Kan kültürü cihazındaki pozitif sinyal süreleri en kısa yedi saat 55 dakika, en uzun 31 saat 22 dakika, ortalama pozitiflik süresi 12 saat 55 dakika olarak belirlendi. Kültür sonrası MALDI TOF identifikasyonu ile LFM identifikasyonun duyarlılığı %82 (32/39), mikroarray yönteminin ise duyarlılığı %87.1 olarak hesaplandı. Lizis filtrasyon sonrası uygulanan MALDI TOF ile mikroarray yöntemlerinin birbirleri ile benzerliği %79.4 (31/39), her üç yöntemin birlikte tutarlı sonuç verdiği örneklerin oranı ise %76.9 (30/39) olarak hesaplandı. En sık saptanan üç etken Klebsiella spp, Acinetobacter spp. ve Escherichia spp. kültür sonrası MALDI TOF ve LFM yöntemi ile uyumu Cohen’in kappa katsayısı sırası ile 0.715, 0.843, 0.938; mikroarray yöntemi ile uyumu 0.935, 0.753, 0.938 olarak hesaplandı ve her üç bakteri için de yüksek uyumluluk elde edildi. Sonuç: Elde edilen verilere dayanılarak, hem lizis filtrasyon hem de mikroarray platformunun sepsis tanısını kısaltmaları açısından yararlı olduğu düşünülmüştür. Lizis filtrasyon yöntemi maliyet etkinlik açısından önde bulunmuştur.
Purpose: In cases of sepsis, death rates are increased per hour of loss in cases receiving or not receiving incorrect antimicrobial treatment. In this study, the aim is to compare the conventional blood culture/MALDI TOF method with the MALDI TOF MS (LFM) and the microarray method after lizis filtration in order to identify the factor faster than the conventional culture method. Method: between November 2015-February 2016 Ege University, Medical Faculty Hospital, Adult Intensive Care units taken from patients in blood culture samples of 39 grams of negative influencer microarray and LFM process was applied and then studied with the MALDI TOF system. The results of the MALDI TOF and microarray testing methods applied after lizis filtration were compared with the MALDI TOF type-level identification made from the bacterial colony. Results: The positive signal time in the blood culture device was set as short as seven hours 55 minutes, the longest 31 hours 22 minutes, the average positive time 12 hours 55 minutes. The post-cultural MALDI TOF identification and LFM identification sensitivity were calculated at 82% (32/39), while the microarray method sensitivity was calculated at 87.1%. The likelihood of the MALDI TOF and microarray methods applied after Lizis filtration was 79.4 % (31/39), while the proportion of samples given consistent results by both three methods was 76.9 % (30/39) calculated. The three most frequently detected factors are Klebsiella spp, Acinetobacter spp. and Escherichia spp. Compatibility with the post-cultural MALDI TOF and LFM method was calculated as 0.715, 0.843, 0.938; Compatibility with the microarray method was calculated as 0.935, 0.753, 0.938 and a high compatibility was achieved for both three bacteria. The result: Based on the obtained data, it is believed that both lizis filtration and microarray platforms are useful in order to shorten sepsis diagnosis. The Lizis filtration method has been leading in terms of cost efficiency.
