Amaç: Meme kanserinin evrelemesinde Fluorine‐18 florodeoksiglukoz pozitron emisyon tomografi (PET/BT) özellikle uzak metastazların saptanması için kullanılmasının yanında bölgesel durumu değerlendirmede de kullanılmaktadır. PET/BT'nin hem aksillayı hem de tümörün reseptörsel durumunu değerlendiren çalışmalarda çelişkili sonuçlar görülmektedir. Bu çalışmada, PET/BT'nin hem aksillayı hem de primer tümörün reseptörsel durumu değerledirmedeki etkinliğinin incelenmesi amaçlandı. Gereç ve Yöntem: Ocak 2015-Haziran 2017 tarihleri arasında meme kanseri tanısı almış ve neoadjuvan tedavi almadan PET/BT çalışılan 75 hasta çalışmaya dahil edildi. Hastaların tüm verileri retrospektif olarak incelendi. Uzak metastazı olan, aksilla dışı lenf nodu metastazı olan ve PET/BT öncesi neoadjuvan tedavi alan hastalar çalışma dışı bırakıldı. Bulgular: Çalışmadaki 75 hastanın 53 ünde histopatolojik olarak lenf nodu metastazı varken 22 tanesinde yoktu. PET/BT sonuçlarına göre 36 hasta pozitif ve 39' u negatif idi. PET/BT'nin aksillayı değerlendirmedeki sensitivitesi %62.3 spesifitesi %86.3 doğruluk %69.3 pozitif prediktif değer %91.6 ve negatif prediktif değer %48.7 idi. Luminal gruplar arasında SUVmax açısından anlamlı farklılık izlenmedi. Tümördeki östrojen ve progesteron reseptörlerinin pozitifliği arttıkça SUVmax azalıyordu. Sonuç: PET/BT'nin aksillayı değerlendirmedeki spesifitesinin yüksek olmasına rağmen sensitivitesi yeterli seviyede bulunamamıştır. Ayrıca primer tümörün SUVmax değeri histolojik reseptörler açısından farklılık sergilese de tahmin etmek için yeterli değildir.
Purpose: Fluorine-18 in the development of breast cancer is also used in the regional state assessment, in addition to its use for the detection of remote metastases (PET/BT). Studies that assess both the acillus and the receptive state of the tumor of PET/BT show contradictory results. In this study, it was aimed at studying the effectiveness of PET/BT in assessing both acillus and primary tumor receptor status. Instruments and Methods: 75 patients who were diagnosed with breast cancer between January 2015 and June 2017 were included in the study and PET/BT were studied without neoadjuvan treatment. The patient’s data was retrospective. Patients with distant metastases, non-acillary lymph nod metastases and pre-PET/BT neoadjuvan treatment were dismissed. Results: 75 of the patients in the study had 53 histopathological lymphatic nod metastases, while 22 were not. According to the PET/BT results, 36 patients were positive and 39 were negative. PET/BT’s sensitivity in the accillation assessment was 62.3 percent specificity 86.3 percent accuracy 69.3 percent positive predictive value 91.6 percent and negative predictive value 48.7 percent. There was no significant difference between the Luminal groups in terms of SUVmax. As the positivity of the estrogen and progesterone receptors in the tumor increased, the SUVmax decreased. The result: Despite the high specificity of PET/BT in the assessment of the axility, its sensitivity has not been found at a sufficient level. Also, the SUVmax value of the primary tumor is not enough to predict even if it shows a difference in terms of histological receptors.
