Erken Dönem Akut Solunum Sıkıntısı Sendromlu Hastalarda Metilprednizolon Tedavisinin Etkileri

Amaç: Yüksek insidansı ve kötü sonuçlarına rağmen, akut solunum sıkıntısı sendromunda (ARDS) özel bir tedavi yoktur. Çeşitli çalışmalarda birçok farmakolojik ajan araştırılmıştır, ancak etkinliği kanıtlanmış tedavi seçenekleri sınırlıdır. Çalışmamızın amacı, erken dönem ARDS'de metilprednizonon tedavisinin mortalite ve morbidite üzerine etkisini belirlemektir. Yöntemler: Ocak 2008 - Aralık 2012 tarihleri arasında mekanik ventilasyon (MV) uygulanan 47 ARDS'li hasta bu çalışmaya dâhil edildi. Hastalar, metilprednizolon grubu (22 hasta) ve kontrol grubu (25 hasta) olmak üzere iki gruba ayrıldı. Metilprednizolon grubundaki kişiler ilk gün 2 mg/ kg intravenöz yükleme dozunu takiben, 2-15. günlerde 6 saatte bir 0,5 mg/kg ve 16-22 günlerinde her 6 saatte 0,25 mg/kg'lık metilprednizolon infüzyonu aldılar. Bulgular: Kontrol grubu ile karşılaştırıldığında, metilprednizolon grubunda MV'den ayrılma oranı anlamlı olarak daha yüksek (p=0,005), MV süresi daha kısa (p=0,021) ve mortalite oranı daha düşük idi (p=0,013). Kaplan Meier analizinde, metilprednizolon grubundaki sağkalım olasılıkları kontrol grubuna göre anlamlı derecede daha yüksekti (p=0,022). Ek olarak, metilprednizolon grubunda akciğer hasarı skoru, çoklu organ disfonksiyon sendromu skoru ve C-reaktif protein değerleri daha düşüktü. Ancak, her iki grup arasında ARDS’nin geliştiği gün, ARDS süresi ve hastanede kalış süresi bakımından farklılık yoktu. Sonuç: Çalışmamız ARDS tanısı alan hastalarda, erken metilprednizolon tedavisinin pulmoner ve ekstra pulmoner organ fonksiyonlarında anlamlı düzelme sağladığını, MV'den ayrılma olasılığını arttırdığını ve mortaliteyi azalttığını göstermiştir.

Early Period Acute Respiratory Disorder Effects of Methylprednisolone Treatment in Patients with Syndrome

Purpose: Despite its high incidence and bad results, there is no specific treatment for the acute respiratory disorder (ARDS). Many pharmacological agents have been studied in a variety of studies, but the treatment options that have been proven to be effective are limited. The aim of our study is to determine the effect of methylprednison therapy in early ARDS on mortality and morbidity. Methods: 47 patients with ARDS who were applied mechanical ventilation (MV) between January 2008 and December 2012 were included in this study. Patients were divided into two groups: the methylprednisolone group (22 patients) and the control group (25 patients). Persons in the methylprednisolone group after the dose of 2 mg/kg of intravenous load on the first day, 2-15. They were given an infusion of 0.5 mg/kg every 6 hours of the day and 0.25 mg/kg of methylprednisolone every 6 hours of the 16-22 days. Results: Compared to the control group, the rate of deviation from the MV in the methylprednisolone group was significantly higher (p=0,005), the MV time was shorter (p=0,021) and the mortality rate was lower (p=0,013). In the analysis of Kaplan Meier, the chances of survival in the methylprednisolone group were significantly higher than the control group (p=0,022). In addition, the result of lung damage in the methylprednisolone group was lower, the result of multiple organ dysfunction syndrome and the C-reactive protein values were lower. However, there was no difference between the two groups on the day the ARDS developed, the time of the ARDS and the time of stay in the hospital. Result: In patients diagnosed with ARDS, our study has shown that early methylprednisolone treatment provides significant improvement in the pulmonary and extra pulmonary organ functions, increases the likelihood of departure from the MV and reduces mortality.
