Ekstra-anatomik baypas cerrahisinde orta ve uzun dönem sonuçlarımız

Abstract:

Aim: Extra-anatomic bypass procedures are alternative applications that attempts to rescue the limb. In this study, the intermediate and long-term results of the patients who underwent extra-anatomic bypass were evaluated.Materials and Methods: Thirty-two patients underwent extra-anatomic bypass procedures in our clinic between January 2004 and January 2010 were reviewed retrospectively. The patients’ data were obtained from the hospital automation program, polyclinic controls, and telephone calls.Twenty seven patients were male and fi ve patients were female. The mean age of the patients was 62±12 years (range, 33 to 88 years). Thirty one extra-anatomic bypass revascularizations were performed for the lower extremities. One revascularization was performed for the upper extremities. Twenty two cases underwent femoro-femoral bypass, eigth cases underwent axillo-bifemoral bypass, two cases underwent axillo-femoral bypass, one case underwent carotico-brachial bypass. Totally 34 ekstra-anatomik bypass procedures were performed for 32 patients. Ring polytetrafl uoroethylene (PTFE) graft was used for all of the operations.Results: The average follow-up of the patients was 28.8±16.7 months (6-74 months). During this period in 24 of the patients graft patency was detected to be continued. Four patients in the postoperative period, two patients in the long follow up period died due to the concomitant diseases. In the postoperative period graft infection was developped in three patients, graft thrombosis occurred in four patients and two patients underwent amputation. Conclusion: Extra-anatomic bypass procedure is an alternative surgical approach type due to its low mortality and morbidity rates, especially in the risky patients population with ischemic leg. In the early and long-term period graft patency can be kept in high rates with proper surgical techniques and medical treatment support
