Periferik Vestibüler Hastalıklarda o-VEMP ve v-HIT Test Sonuçlarının Kalorik Test Sonuçları İle Karşılaştırılması

Amaç: Periferik vestibüler hastalıklarda oküler vestibüler uyarılmış miyojenik potansiyel (o-VEMP) ve video head impulse (v-HIT) test sonuçlarını kalorik test sonuçları ile karşılaştırmak. Vertigolu hastaların değerlendirilmesinde test sıralaması içim bir algoritma oluşturmak. Yöntemler: Baş dönmesi şikayeti ile müracaat eden hastaların tam bir kulak burun boğaz muayenesi yapıldıktan sonra hastaların videonistagmografi, o-VEMP ve v-HIT testlerinin yapılmış ve hepsinin temporal MR tetkikleri yapılmıştır. Odyovestibüler inceleme ve radyolojik görüntüleme sonrasında santral patoloji düşünülen hastalar, benign paroksismal pozisyonel vertigosu olan hastalar ve vestibüler migren tanısı konulan hastalar ve planlanan test bataryasının uygulanamadığı hastalar çalışma dışı bırakılmıştır. Çalışmaya kriterleri sağlayan ilk 30 hasta dahil edilmiştir. Bulgular: Hastaların 22’sinde (%73,3) kanal paralizi tespit edilmiştir. On dokuz hastada (%63,3) VEMP testi patolojik olarak tespit edilirken, v-HIT testi ise sadece 8 hastada (%26,7) patolojik olarak tespit edilmiştir. Kanal paralizi olan 22 hastanın 17 sinde (%77,3) VEMP +/- v-HIT patolojisi mevcuttur. İki test birlikte ele alındığında vestibüler hipofonksiyonu tespit etmede sensitivite %77,3 spesivite ise %80,1 olarak bulunmuştur. Sonuç: VEMP testi ve v-HIT testi vestibüler hipofonksiyonu belirlemede kalorik testin yerini almaya yeterli olabilecek düzeyde gözükmemektedir. Ancak her iki test birlikte vertigolu hastanın değerlendirmesinde başlangıç testi olarak kullanılabilir. İki testten herhangi biri patolojik elde edildiğinde kalorik test yapmaya gerek olmayacaktır.

Purpose: Compare the outcomes of the ocular vestibular stimulated myogenic potential (o-VEMP) and video head impulse (v-HIT) test with the calorie test results. In the assessment of vertigolic patients, the test ranking creates an algorithm. Methods: After a full ear and nasal throat examination of patients with a headache complaint, patients have been performed video-istagmography, o-VEMP and v-HIT tests and all of them have been performed temporary MR examinations. Patients with central pathology considered after odyovestibular examination and radiological imaging, patients with benign paroxysmal positional vertigus and patients diagnosed with vestibular migraine and patients where the planned test battery cannot be applied have been dismissed. The first 30 patients provided criteria were included in the study. Results: In 22 patients (73.3%), canal paralysis was detected. In nine patients (63.3%), the VEMP test was pathologically detected, while the v-HIT test was patologically detected in only 8 patients (26.7%). In 17 of 22 patients with canal paralysis (77.3%), VEMP +/- v-HIT pathology is present. When the two tests were handled together, sensitivity was 77.3 percent specificity and 80.1 percent sensitivity in vestibular hypofunction. The result: the VEMP test and the v-HIT test appear not to be sufficient to replace the calorie test in determining vestibular hypofunction. However, both tests can be used together as a starting test in the evaluation of a vertigual patient. When any of the two tests are achieved pathologically, there will be no need to do calorie tests.
