Travma Sonrasında İnterhemisferik Subdural Hematomu Olan Hasta: Olgu Sunumu

Travmatik interhemisferik subdural hematom (ISH) nadir görülen önemli bir durumdur. ISH tüm travmatik subdural hematomlu hastaların yaklaşık %6’sını içerir. Bizim vakamızda araç içi trafik kazası ile acil servise getirilen 48 yaşında erkek hasta sunulmuştur. Nörolojik muayenesinde patolojik bulgu ve KIBAS bulguları yok idi. Beyin tomografisinde falx düzeyinde anteriorda en geniş yerinde 13 mm’ ye varan lineer hiperdens görünüm (subdural hemoraji) tespit edildi. Travma hastalarında nörolojik muayenede patolojik bir bulgusu olmadan, kanamaya yönelik herhangi bir risk faktörü olmasa da kafa içine kanama gerçekleşebilir. Bu nedenle travma öyküsü olan hastalarda kranial görüntüleme istenmesi uygun olur.

Abstract:

Traumatic interhemispheric subdural hematom (ISH) is a rare significant condition. ISH includes approximately 6% of all traumatic subdural hematomic patients. A 48-year-old male patient was brought to emergency service with a car accident in our case. In the neurological examination there were no pathological findings and KIBAS findings. In the brain tomography, a linear hypertension appearance (subdural hemorrhage) was detected at the falx level at the widest location of the front to 13 mm. In patients with trauma, no pathological findings in the neurological examination may occur, although there is no risk factor for bleeding. Therefore, it is advisable that patients with a trauma history are requested a cranio screening.
