Amaç: İlaçların ruhsat aldıkları alan dışındaki farklı endikasyon, doz, uygulama şekli, yaş grubu, özel popülasyon vb. kullanımları “endikasyon dışı ilaç kullanımı” (EDİK) olarak adlandırılır. Bu çalışmada EDİK'in Türkiye’de farklı iller ve bölgeler düzeyinde incelenmesi amaçlandı. Metot: 2011 yılında Türkiye İlaç ve Tıbbi Cihaz Kurumuna EDİK başvurusu yapılmış toplam 5681 başvuru kaydı incelendi. Bu başvurular il ve NUTS-1 (Nomenclature of Territorial Units for Statistics) bölgesi düzeyinde; hastaların yaşı, cinsiyeti, başvurunun yeri, zamanı vb. özellikleri ve her bölgede en sık başvurusu yapılan ilk beş ilaç bakımından ayrıntılı olarak değerlendirildi. Bulgular: EDİK başvurularının %81,6’sının üniversite hastanelerinden geldiği ve başvuruların daha çok takvim yılının 2. çeyreğinde yoğunlaştığı saptandı. Güneydoğu Anadolu Bölgesi hariç diğer tüm NUTS-1 bölgelerinde başvurusu yapılan hastaların yaş ortalamasının 30’un üstünde olduğu ve hastaların çoğunluğunu (%55,1) kadınların oluşturduğu tespit edildi. Kurumsal düzeyde EDİK başvurularının en sık Batı Anadolu Bölgesinden (%34,9) yapıldığı ve bunu İstanbul (%26,5) ve Ege (%10,3) bölgelerinin izlediği belirlendi. İller bazında, her 1000 uzman hekim başına düşen başvuru oranı en fazla Çankırı (778,8), en az ise İstanbul’da (20,0) saptandı. Her bir bölgede en sık EDİK başvurusu yapılan ilaçlar incelendiğinde, sıralaması bölgeler arasında değişmekle beraber en sık karşılaşılan ilaçların “rituksimab”, “mikofenolat mofetil” ve “iloprost trometamol” olduğu bulundu. Sonuç: EDİK'in rasyonel kullanımı yerel özellikler de dahil bazı kritik faktörlere bağlıdır. EDİK başvurularının iller ve bölgeler düzeyindeki farklılıkları ilk kez bu çalışma ile gösterilmiştir. EDİK’in işlevselliğinin arttırılmasında bu farklılıkların göz önünde bulundurulması gerekir. Elde edilen bu tespitlerin, söz konusu farklılıkların nedenlerini doğrudan ortaya koyacak yeni çalışmalara ve düzenlemelere ışık tutması beklenmektedir.
Purpose: Different indications, doses, methods of application, age group, specific population, etc. Their use is called "non-indicated drug use" (EDIC). In this study, EDIK was intended to be studied in different provinces and regions in Turkey. Method: In 2011 a total of 5681 applications for EDIK applications to the Turkish Medicine and Medical Equipment Institute were examined. These applications are at the NUTS-1 (Nomenclature of Territorial Units for Statistics) regional level; the patient’s age, gender, place, time of application, etc. It was detailed in terms of its characteristics and the first five most frequently applied medications in each region. Results: 81.6% of EDIC applications come from university hospitals and the number of applications is 2 calendar year. It was concentrated in the quarter. The average age of patients applied in all other NUTS-1 regions except in the southeastern Anadolu region was over 30 and the majority of patients (55.1%) were women. On corporate level, the most frequent EDIK applications were made from the West Anadolu Region (34.9%) and followed by the regions of Istanbul (26.5%) and Ege (10.3%). On the basis of the insights, the application rate per 1000 specialist doctors was the most Chankırı (778,8), and at least in Istanbul (20,0) was determined. When the most frequently applied medications in each region were examined, the rankings varied between regions and the most frequently found medications were "rituksimab", "mikofenolate mofetil" and "iloprost trometamol". Result: The rational use of EDIK depends on some critical factors, including local characteristics. The differences in the counties and regions of the EDIC applications were first shown by this study. In increasing the functionality of the EDIC, these differences should be taken into account. These findings are expected to keep light on new studies and regulations that will directly reveal the causes of these differences.
