Amaç: Bu çalışmanın amacı; göğüs hastalıkları kliniğinde venöz kan gazlarının (VKG), arter kan gazları (AKG) yerine kullanılabilirliğini araştırmaktır. Materyal ve Metot: Bu kesitsel çalışma Aralık 2017- Mayıs 2018 tarihleri arasında Göğüs Hastalıkları Kliniği’nde yapıldı. Çalışmaya primer akciğer hastalıkları nedeni ile solunum yetmezliği gelişen 86 hasta dahil edildi. Olgulardan eş zamanlı AKG ve VKG alındı ve karşılaştırıldı. Olgularda; arter ve venöz kan gazlarında pH, parsiyel karbondioksit basıncı (pCO2), parsiyel oksijen basıncı (pO2), oksijen saturasyonu (SO2) ve bikarbonat (HCO3) ilişkisi araştırıldı. Bulgular: Çalışmaya dahil edilen 86 olgunun yaş ortalaması 70,89±15,54’di. Olguların 36’sı (%41,90) kadın, 50’si (%58,10) erkekti. 66’sı (%76,70) tip 1, 20’si (%23,30) tip 2 solunum yetmezliğine sahipti. Arter-venöz kan gazlarında pH, pCO2, HCO3, değerlerinde pozitif korelasyon mevcuttu (sırasıyla; r=0,780, p<0,001; r=0,880, p<0,001; r=0,817, p<0,001). pH için ortalama fark 0,03±0,76, %95 CI [-0,12-0,19], pCO2 için ortalama fark -5,45±7,15, %95 CI [-6,98-(-3,92)], HCO3 için ortalama fark -0,99±3,90 %95 CI [-1,83-(-0,15)] bulundu. PO2 ve SO2 değerlerinde korelasyon saptanmadı (sırasıyla; r=0,167, p=0,124; r=0,079, p=0,472). Sonuç: Bu çalışmada, arter ve venöz kan gazlarında, pH, pCO2 ve HCO3 değerlerinde pozitif korelasyon gözlenmiştir. Bu bulgular göğüs hastalıkları kliniğinde solunum yetmezliği nedeni ile izlenen olgularda asit baz dengesini değerlendirmede, VKG’nin AKG yerine kullanılabileceğini düşündürmektedir.
Purpose: The purpose of this study is to investigate the availability of venous blood gases (VKGs) in the clinic of breast diseases, instead of arterial blood gases (AKGs). Material and Method: This cutting study was conducted in the Breast Disease Clinic between December 2017 and May 2018. The study included 86 patients developing respiratory failure due to primary lung diseases. The events of AKG and VKG were taken and compared. In cases; the relationship between the pH, the particle carbon dioxide pressure (pCO2), the particle oxygen pressure (pO2), the oxygen saturation (SO2) and bikarbonate (HCO3) in the artery and venous blood gases was studied. Results: The average age of 86 incidents included in the study was 70,89±15,54. 36 of the cases (41.90%) were women and 50 (58.10%) were men. 66 (76.70%) of type 1, 20 (23.30%) had type 2 respiratory failure. In arterial and venous blood gases there was a positive correlation in the values of pH, pCO2, HCO3 (r=0,780, p<0,001; r=0,880, p<0,001; r=0,817, p<0,001). The average difference for pH was 0,03±0,76, 95% CI [-0,12-0,19], the average difference for pCO2 -5,45±7,15, 95% CI [-6,98-(-3,92)], the average difference for HCO3 -0,99±3,90% CI [-1,83-(-0,15)]. No correlation was found in PO2 and SO2 values (r=0,167, p=0,124; r=0,079, p=0,472). Result: In this study, positive correlation in arteric and venous blood gases, pH, pCO2 and HCO3 values was observed. These findings suggest that in assessing the acid-based balance in cases observed with the cause of respiratory failure in the breast disease clinic, VKG can be used in place of AKG.
