Abstract:

Aıim: The purpose of the study was to evaluate the elinical validity the urinary tripsinogen-2 strip test in the diagnosis of acute pancreatitis. Methods: The prospective study consisted of 200 consecutiue patients admitted to the emergency unit with acute abdominal pain. The urinary and blood samples of these patients were analyzed for urinary and blood amylase and urinary tripsinogen-2 levels. The results were evaluated for sensitivity, specificity, positive and negative predictive values, accuracy rate and relative risk ratio. Continous data were compared by the Mann-Whitney U test and test strip results were compared by McNemar's test. Results: In 16 of the 200 patierıts with abdominal pain, the diagnosis of acute pancreatitis could be established. The sensitivity of the urinary tripsinogen-2 strip test, urinary and serum amylase results were 98%, 81%, 88% and specificity were 95%, 94% and 93%, respectively. The positive and negative predictive values were 61%, 54%, 52% and 99%, 98%, 99%, respectively. The accuracy rate and relative risk ratio of these tests were 95%, 93% and 53.87, 31. 78, 44.85 respectively. Conclusions: The three tests studied were not significantly different from each other in terms of the parometers taken into account. But the urinary tripsinogen-2 strip test can be considered a valuable non invasive. simple urinary screening test in the diagnosis of acute pancreatitis especialy in subelinical cases.
